Please find attached a copy of the EFT Authorization form that the client completed the day she joined. We have her signature in placesShe also check off on the box that states that client has read, understood and agreed to the terms of the service. We do this so
many times to make sure client has read it. She authorized us to draft her account. The terms are stated for a reason, if client chooses to not read it , there is nothing that we can do about that.. We can not make clients read it. The account is still open. We emailed a copy of her EFT to her on 6-19. She will need to cancel per the terms on the agreement. People just give out all their information and don't even know what they are signing up for. It's not even in small print and is not that much to read! There will be no refund issued as the account was authorized by client for us to draft. Unfortunately for security purposes we have to require either a personal visit to a store location with identification or a written letter in order to cancel the bank draft transaction. letter on 8/17/16 and per the attached notice, have cancelled his account and bank draft. No further funds will be drafted from his account as we received the letter before our cancellation cut off date of the 25th.We appreciate Mr. [redacted]'s business with ULTRATAN and would enjoy having him as a customer again in the future.

On 4/7 Ms. [redacted] signed up for an Ultratan Platinum rewards package. Her package for April was prorated and she paid 47.97 for the remainder of the month. would come out of her account, as well as how to cancel the membership if she did not wish to be drafted for an additional month. Ms. [redacted] used the tanning services throughout April and has also used the service in May as shown on the attached history. We have cancelled her account as requested so she will not be drafted going forward. The proof of cancellation is also attached. She is welcome to continue using the package until her current month expires on 5/31/16. We make every effort to be clear with customers as how our packages work and are disappointed to lose Ms. [redacted] as a customer.
