Patient come in for a wellness visit on January 11, She was asked to come back a month later to review her wellness labs, due to her deficiency in Vitamin DOn January 25, she returned to our office for a follow up visit to review the Vitamin D deficiency and treatment optionsAt that visit the provider ordered a Dexa Scan (bone density scan) to evaluate her bone density and rule out osteoporosisHer insurance applied the visit towards her deductible since there were issues involved and it was NOT a wellness examWe have a commitment to submit all ICD-codes, that were discovered and reviewed with the patient, to the insurance company that we are contracted withThe insurance company determines what goes applied toward their covered wellness exam and what is applied to the patient's deductibleThat is not for us to determineUnfortunately, Mrs [redacted] last visit was applied to her deductible due to the issues that were discussed.Please let me know if you need any further information in regards to this matter [redacted] Practice Administrator Porter Medical Associates

Thats all lies I NEVER asked them for allergy test all I did ask is my blood pressure test thats all , if theres a documents or whatever they sent its not true they made me sign a LOT of paperworks and I just had no time to review every single paper , they are cheaters and liars I did not ask for allergy test again thanks

To whom it may concern, Please allow this correspondence to serve as a response to the above referenced notice of complaintI apologize that I nor my staff ever received an email prior to this letter notification that was received in the mail on September 15, On May 27, Ahmed M [redacted] was evaluated at Porter Medical Associates for allergy testing at his requestThe front desk clerk explained that he had a deductible for the testingHowever, she would not collect any payment from him on that dayInstead, we would bill the insurance first and see what his insurance would leave to his responsibilityHe signed the verification form, which showed the breakdown of his benefits before having the allergy test administeredOnce the test was administered the results were reviewed with MrM [redacted] MrM [redacted] decided to move forward with the immunotherapy treatment and signed another form indicating his approval (see attached)On June 16, our billing department called to speak with MrM [redacted] regarding his balanceOscar Insurance applied $towards his deductible for both the allergy testing and immunotherapyHowever, we gave him a rather large courtesy discount and only billed him $total for the both (this amount has since then been corrected to $total balance due to an error in the amount of discount applied to the allergy testing) which is essentially our costThis discount was given to ensure cost barriers could be overcome and for the patients welfareHe stated that he thought his plan with Oscar covered everything at 100%Our billing clerk explained that she would work with him on payments as well as already giving him a courtesy write offHe went on to say that he was not able to pay anything at allOn August 22, our billing department called MrM [redacted] again to discuss payment arrangementsHe stated that he thought DrPorter was going to "take care of the test" and he could not pay anythingOur last discussion regarding this balance was with Oscar Insurance on August 23, Our billing clerk spoke with a representative that called our office to inquire about the allergy testing and immunotherapyShe stated that the patient had called her and explained that Porter Medical Associates was billing him for a service that he never receivedOnce explaining the situation and showing proof of the testing with his signature she agreed and noted the account at Oscar Insurance.For your convenience, all the above conversations were noted on the account and are attached to this claim for your reviewAll notes on accounts are time and date stamped and cannot be altered after they are entered into our EMR system, Advanced MDAlso attached is the note made on May into MrM [redacted] 's account by our Practice Administrator, [redacted] , stating that the patient understood that he would be responsible for up to $for the immunotherapy and $for allergy testing.Neither I nor my staff would ever give any insurance information to any patientThe information that we present to the patient is given to our staff directly from the patients insurance company and is recorded with a reference number for verificationThe verification of benefits for all testing administered by Porter Medical Associates is reviewed by our staff as a courtesy to our patientsHowever, the ultimate responsibility lies in the hands of the policy holderIn this case we sincerely wanted to give MrM [redacted] , not only excellent medical care but go above and beyond to discount the service to what is essentially our cost to provide the serviceMrM [redacted] was given notice several times of his potential financial responsibilityProfessional services and equipment, as well as consumable supplies, were used on this patient, now apparently all at my costThe accusation of "Billing or Collection Issues" simply is unsupportedAll proper CPT codes with supporting ICD-diagnosis codes were utilizedIn addition, services were performed as stated and billed (see enclosures).The fact that a patient would be told that nothing would ever be expected in payment is absolutely absurd; in fact this would be fiscally reckless of any small business to operate in such a mannerIT seems in retrospect that this patient is happy to receive any and all services desired only as long as a third party foots the bill.I have practiced medicine in the State of Texas for over years and am reputedly known for the quality of care that I give each individual patientObviously, it is hoped that your investigation will reveal that this complaint lacks merit and will be dismissed appropriatelyPlease contact us at you earliest convenience should you need any further information

I am rejecting this response because:The doctor did not relay information on further testing in that mannerThe doctor just said it was something he was recommendingNever did the doctor say it was beyond the scope of the wellness examI kept informing the doctor that I only wanted the wellness exam, and I did keep asking about cost

Oscar Insurance applied $4215.00 towards his deductible for both the allergy testing and immunotherapy. However, we gave him a rather large courtesy discount and only billed him $800 total for the both (this amount has since then been corrected to $600 total balance due to an error in the amount of discount applied to the allergy testing) which is essentially our cost. This discount was given to ensure cost barriers could be overcome and for the patients welfare. He stated that he thought his plan with Oscar covered everything at 100%. Our billing clerk explained that she would work with him on payments as well as already giving him a courtesy write off. He went on to say that he was not able to pay anything at all. On August 22, 2016 our billing department called Mr. M[redacted] again to discuss payment arrangements. He stated that he thought Dr. Porter was going to "take care of the test" and he could not pay anything. Our last discussion regarding this balance was with Oscar Insurance on August 23, 2016. Our billing clerk spoke with a representative that called our office to inquire about the allergy testing and immunotherapy. She stated that the patient had called her and explained that Porter Medical Associates was billing him for a service that he never received.
