13, October, 2013Dear *** ***:The attached letter was referred to my offices on 3, October, The letter indicates that as of 29, September, neither the patient or their attorney received a copy of their itemized billThe following actions occurred:1st, Our records
indicate that the patient's attorney, requested an itemized bill be mailed to the patientat the address given to the registration clerk in the Emergency DepartmentA detailed bill was mailed to the resident's given address on 07/16/Our records indicate the patient's request was fulfilled in a timely manner.2ndOur records indicate receipt of a letter from the patient's attorney on 07/17/Our recordsindicate that an itemized bill was faxed to his offices at *** * *** or 07/17/2014.3rdOur records do not indicate receiving numerous calls or visits to our office by the patient (either mother or son), but our records do indicate that collection representatives of the hospital have made numerous efforts to contact and resolve these outstanding balances, without success.When the patient presented for services, they assigned insurance benefit payment to the hospitalThey did not provide insurance at the timeAs insurance benefits were assigned to the hospital, and the patient did not provide insurance, we are unable to bill an insurance, Billing to an insurance requires a detailed bill and codes for rendered servicesThe patient’s complaint is that we will not provide this information to her and are calling HerIt would appear that this individual is seeking to bill and collect from her insurance company and not honoring her obligation to the hospitalIf she provides her insurance information to the hospital, we would bill on her behalf and satisfy her bill for services providedShe is choosing not to do soIt appears she is seeking coding information from our Medical Records Department, not Patient Financial ServicesPatient Financial Services cannot release diagnosis or coding information on a claimI would recommend that this individual provide billing information for her insurance so we may bill on her behalf arid satisfy as much of her financial obligation as permissible under her benefitsTo date, this individual has been unwilling to comply with this usual and customary request.Regards,

It wasn't at this location that I was seen at but the Clinton office I had my mom with me who have dementia I make are appointments at the same time I saw a dr d[redacted] there we arrive at our appointment 30 minutes early he was seeing a patient they took me and my mom back at 1:06pm our appointment was at 1pm they left us in the room we caught metro access there they was due to pick us up at 1:55pm I went to schedule the next appointment for us why we was waiting on dr d[redacted] he was in the office eating a apple talking I ask him was he given me my injection because when been back there for over 20 minutes waiting he come back there and said to me that he is not on my time he is not going to see me that I was rude and to get out of the office he was yelling this inthe hallway while I was in the exam room I had already told him that I was not going to see him today because we was getting ready to be picked up by metro access I asked the staff for the medical borad number they act like they was scared but I got everything under control I'm going to notify them as well as fox 5

Review: My son and I went to this location on January 27, 2014 in connection with a car accident ([redacted]XX and [redacted]XX). We were seen by the doctor and inform them that this visit was do to an car accident. I have been contacting them went by and left numerous messages to be contacted back regarding I need an itemize bill showing the codes of of treatment and the fees has been paid in order to receive this information. As of today (September 29, 2014) I nor my lawyer office has recieved anything back from United Medical Center. There is never a human being aswering the phones you will always get a automatic voicemail saying she is away from her desk to leave a message I cant begain to tell you how many messages I and my lawyer has left. And when you dial for operator all they do is transfer you back to the voicemail and was told thats the best they can do. They stated that medical record has it own separte department and they are the only ones that can assist me. But whats crazy to me is that my son and I are constantlly received threatening phones calls regarding our outstanding bills along with mail to follow to request payment. We are trying to handle our bill but we cant tell we receive what we need to get them paid from the auto accident. ThanksDesired Settlement: I need a complete billing details along with codes sent to me and or my lawyer my address is attached thanks .
