Complaint: [redacted] I am rejecting this response because I did not talked to them and aurthorized this plan it was done throught an health plan angency and I thought I was canceling the whole policy when I canceled the policy for health they told me if I did not like the policy just cancel I did not know the dental plan was separate I was not told that if so I would have canceled it to because I canceled the health plan the next weak after I got it this insurance thing is new to me I am newly retired after yrsnever had to search for insurance I am years old not trying to take anything from anydody just want to be treated right did not know I had dental insurance if I did I would have used it never used it I think I deserve a refund Regards, [redacted] [redacted]

Ms [redacted] applied for membership with United Service Association on July 24, At time of enrollment, her membership included a Member Reward ProgramIn 2014, some of the benefits offered in the memership were cancelled, resulting in a decrease in the monthly dues that were withdrawn from Ms [redacted] 's account Correspondence advising of the changes that were made were mailed to Ms [redacted] Mr [redacted] stated today (August 17, 2017) that they received a letter regarding the change in the monthly cost, but did not receive correspondence regarding the Member Reward ProgramUnited Sevice Association will honor the Member Reward Program that terminated on June 8, 2014, and a check for $5,will be mailed to Ms [redacted] and should be received by September 15,

The first cancellation request that we received was on April 11, 2018. The membership was immediately cancelled on April 11, 2018. We did not receive a request to cancel prior to April 11,

Ms*** *** applied for a membership with United Service Association on June 29, 2017. At that time, in accordance with our enrollment procedures, which requires either an electronic signature or a voice authorization, Ms*** completed a voice authorization. A copy of the
authorization is accessible via the following link: *** As indicated during the voice verification, Ms*** agreed to an effective date of August 1, In regards to a male representative that Ms*** spoke with that was rude, we do not have any calls from Ms*** between Ms*** and a male employee. Ms*** did speak with our female representatives on July 11, and July 26, 2017. Pursuant to her request that was made on July 26, 2017, her account was cancelled and a refund was issued to her credit card on July 27, We regret any inconvenience or dissatisfaction that Ms*** may have experienced. Ms*** enrolled in the association on June 8, and was issued a Dental Access plan. The plan she purchased includes coverage under a group insurance policy and is not a discount plan. This information was explained to Ms*** in detail. All members are entitled to
a 30-day evaluation period in order to review the materials and determine if the product will suit their needs. Should a member wish to cancel during the evaluation period, a full refund is issued and processing fees nor any other fees are retainedMs*** requested cancellation an in accordance with our policy, a full refund has been issued to her credit cardPlease let me know if you have any questions or need any additional information

I am rejecting this response because:  it was not simply a matter of non use of the services.  We cancelled the services, and it was not cancelled as requested via their third party agent.   The company apparently needs more...

reliable or honest 3rd party agents. The fact that this deduction continued to happen due to us confusing it with another does not absolve them of the responsibility that we cancelled, and they continued to take our money.

Corporation Act (D.C. Code, 1981 Edition,Title 29, Chapter 5) by the Department of Consumer and Regulatory Affairs, Washington, D.C.USAHC provides home, travel, health, entertainment and other benefits to its members.Ms. [redacted] voluntarily enrolled as a member of United Service Association For Health Care(USAHC) on September 28, 2015, presented herself as the authorized card holder and authorized amonthly charge from her account in the amount of $69.00. Subsequent to enrollment, a fulfillmentkit, which includes an ID card and Welcome Letter, was sent to Ms. [redacted]. The ID card includesthe member's name, membership number, the USAHC logo, our toll-free number, and a stickerthat is affixed to the card advising the member to call and register the membership. Theregistration process allows the member to review their account information, benefits and discussany questions or concerns that they may have with one of our representatives.In Ms. [redacted]'s complaint, she indicated that she did not authorize any charges, did not receive anID card in the mail and infers that she believes the call that we have on file is a fake and/ or wasaltered to make it appear that she agreed to the purchase.The call between Ms. [redacted] and our representative, which occurred on October 12, 2015 is attachedto this reply. The conversation between our representative and Ms. [redacted] lasted for nine (9)minutes and 54 seconds. During this call, Ms. [redacted] provided her name, provided her USAHCmembership number, verified her address, verified the last four digits of her account, raisedquestions regarding coverage and was advised that she had 30 days to decide if she wanted toretain the membership or cancel it and receive a full refund.Ms. [redacted] also contacted our office on February 22, 2016. During this call she indicated that shehad a dental plan thru the "Obama Care Network" provided the membership ID number asprinted on her ID card, indicated that an amount of $69 was withdrawn from her account andwanted to know if there was a balance due. Our representative advised Ms. [redacted] that the amountof $69 was automatically withdrawn from her account on the 28th of each month and there was nobalance due.The next call received from Ms. [redacted] was on March 21, 2016. The representative reviewed thecharge of $69 and answered the questions raised by Ms. [redacted] During this call, our representativespecifically asked Ms. [redacted] if she wanted to cancel the membership and she indicated that she didnot wish to cancel.The next call that we received from Ms. [redacted] was on January 3, 2017. The representative advisedMs. [redacted] that we would cancel her account as requested and discontinue drafting her account.Additionally, since the last draft had occurred within five (5) days of her call, the draft wasrefunded to her and based on our review of the information available, no additional refund is due.Unfortunately, Identity theft is prevalent and if Ms. [redacted] believes that someone other than herselfmade the purchase and placed the phone calls, with her permission, we will forward all of theinformation that we have on file to the Oak Hill Police Department on her behalf.We trust that we have provided sufficient information to close this inquiry. above. We trust this answers the questions raised. United Service Association
For Health Care (USA+) is a non-profit corporation chartered under the
provisions of the District of Columbia Non-Profit Corporation Act (D.C Code,
1981 Edition,...

Title 29, Chapter 5) by the Department of Consumer and Regulatory
Affairs, Washington, D.C.USA+ provides home, travel, health, entertainment and other
benefits to its members. Upon joining the association, each member receives a
10-day evaluation period to review the membership and decide if they wish to
continue or cancel the membership. Should a member elect to cancel a
membership, the only requirement is for the member to submit a written request
to do so, via U.S. mail or facsimile, to receive a full refund of their
membership dues and one-time enrollment fee. Requests to cancel after the
initial 30-day evaluation period are also accepted via U.S. mail or facsimile.Mr. [redacted]'s inquiry
indicates that he did not authorize the purchased the Dental 360 Family
membership. As part of USA+ review of the purchase, the Agent of Record
TC34185, Bryan Jarovits provides a written statement that states he sold three
insurance related products to Mr. [redacted]'s spouse, Mrs. [redacted]
(Exhibit B). The one USA+ membership included was the Dental 360 Family
membership. And was authorized via an electronic enrollment and e-signature
authorization. Agent states members called the next day to state they were
"unsatisfied" and wanted to cancel. Agent states he advised they were
entitled to a full refund and they would need to call the number listed on the
welcome email confirmation to request a refund.Enclosed is a copy of the authorization that includes Mrs.
[redacted]'s electronic signature, her email address and IP address that
indicates the computer location on which the enrollment was performed. (Exhibit
A)Upon enrollment USA+ members receive an immediate
acknowledgement email along with instructions on where to log in for their
membership information. Additionally, a welcome letter and membership ID is
also mailed to the member's home redirecting the member to the website for
specific benefit usage1information along with the dental certificate that contain
limitations and exclusion. Along with instructions on how to cancel and request
a refund. Additionally, the second paragraph of the electronic signature
authorization Mrs. [redacted] generated states the 30 day evaluation period and
the third paragraph states clearly:"l understand and acknowledge that the first
month's dues and applicable enrollment fees are collected on the date of
submission regardless of the selected effective date. I agree that your rights
in respect to each such check and electronic debit shall be the same as if it
were a check drawn on you and signed by me. The authority is to remain in
effect until revoked by me in writing, and until you actually receive such
notice. I agree that you shall be fully protected in honoring any such check or
debit".Mr. [redacted]'s spouse enrolled in USA+ and
authorized ongoing drafts, and neither of the [redacted]'s contacted USA+
directly to cancel the membership, until Arthur [redacted] called USA+ on May
03, 2017 to report fraud.USA+ did not commit fraud. Mrs. [redacted] authorized
the membership. In May, USA+ immediately canceled his membership and ceased all
billing of this account when we received such notice.I trust the information provided is sufficient to close this

Ms. [redacted] applied for membership with United Service Association on July 24, 2007.  At time of enrollment, her membership included a Member Reward Program. In 2014, some of the benefits offered in the memership were cancelled, resulting in a decrease in the monthly dues that were...

withdrawn from Ms. [redacted]'s account.  Correspondence advising of the changes that were made were mailed to Ms. [redacted]. Mr. [redacted] stated today (August 17, 2017)  that they received a letter regarding the change in the monthly cost, but did not receive correspondence regarding the Member Reward Program. United Sevice Association will honor the Member Reward Program that terminated on June 8, 2014, and a check for $5,599.00 will be mailed to Ms. [redacted] and should be received by September 15, 2017.

bill, a representative contacted him on January 11, 2016 and advised that we
