In response to Complaint ID# [redacted] , please note [redacted] order for treatment was placed on July 13, On July16th he called to request a cancellation for which we require a signed request form The form was signed July 20th and received by us on July 24th Upon receipt of the form the cancellation was processed and [redacted] was notified While [redacted] remaining treatment payments were cancelled, he still has an outstanding balance of $that covers services rendered and a standard 15% cancellation fee The table below describes the calculation for [redacted] outstanding balance: Balance Calculation Amount Services and Fees $3, Payment Received $2, Outstanding Balance $ Charges and cancellation fee were discussed at the time of sign up Standard procedure was followed in the cancellation of [redacted] treatment and therefore, no additional measures will be taken Should further information be required from Universal Men's Clinic, kindly respond with detailed instructions

Complaint: [redacted] I am rejecting this response because: This is incorrect I called and cancelled in June One July 15th my credit card was charged even though I had not recieved any treatments I called back and they acknowledged they had my treatments had the office and had not sent them to me I was then told they would continue to charge my card because I could only cancel if I came in in person to do so I was then told I could send in a signed letter Brock R [redacted] sent me a letter acknowledging they owed me $and said they would send it to me As of today I have still not recieved it.Sincerely, [redacted]

Dear Ms. [redacted],
This email is in response to Complaint ID #[redacted].
We have been trying to contact the patient directly to resolve this matter to his satisfaction. Unfortunately we have not had any success with email or calls. We understand the patient is refusing to speak to any clinic...

representative because he would like to speak to the company CEO. We would like the patient to understand that the Management Company for the clinic is aware of the situation and has worked out a solution with the clinic. It’s important, however, that the patient communicates with the clinic staff so we can reach resolution.
Neither the clinic nor the Management Company employs “blockers” when working on collections. The clinic staff works with patients directly on all matters from beginning of treatment to the end. We are sorry for any inconvenience the patient experienced when initially communicating his complaint. This has been addressed and coaching has taken place to ensure all communications with patients are clear and pleasant.
We ask that Mr. [redacted] respond to our contact attempts so we can reach an agreement and close this case.
If further information is needed, please do not hesitate to contact me.
