Review: I have tried numerous times (4-5 times) to speak with someone at this business to simply send me a release form so that my personal medical information could be transferred to a different doctor who I am now under the immediate care of. This has been time-sensitive and despite this they seem to not care at all about my needs as a patient. I have called the facility and spoken with numerous people, many of which have promised they would fax and/or email the form but this still not has occurred. They are elusive, and have wasted considerable time. I was to have a procedure which now has to be postponed. This is far more severe than a product issue - this is a human issue.Desired Settlement: I require the immediate assistance of someone in a position of authority at this facility to contact me and take whatever means necessary with their staff or on their own to provide me with this simple but important document. Again, I am in need of a medical procedure and this office has proven extremely careless and cavalier about my needs.
