It was decided that the patient was not fully informed of increase in cost. Additionally, he did not sign a non-covered consent form. Due to these issues, we have removed the requested charges from his account.
This person was refunded the $500 in question; however, he was not refunded the additional amount he states he incurred from the collections agency.
