Dear Representative of The Revdex.com Servicing Metropolitan New York, Thank you for your letter dated June [redacted], 2016. We have reviewed the issue and our findings are as follows: The patient saw the doctor on 5/*/15, and as part of that encounter, she set up an online banking payee account, utilizing a unique account number. The patient saw the doctor again on 11/*/15; this is the encounter in question. It is important to note that this encounter was assigned its own unique account number. The insurance carrier processed the claim and a balance of $15 dollars was left, which was the patient’s responsibility. Unfortunately, when the patient paid this bill online, the payment was forwarded to us with the unique account number from the 5/*/15 date of service, so the payment was not posted to the 11/*/15 date of service, leaving the $15 from the 11/*/15 date of service erroneously outstanding. Unfortunately I found no record of any correspondence or phone calls to our office. Our protocols demand that all interaction with patients, regardless of whether it is a phone call, or correspondence, be notated in our billing system. If the patient has the name of the person she spoke to, it would be helpful for us to ensure that the protocols are properly adhered to. We have correctly posted the payment in question, there is no balance, and no further action is required from the patient. If you need any further help with this issue, please let us know.

I went to Doctor Hanson on 11/**/15. This company charges for a co-payment. I paid $15.00 thru [redacted] on 1/**/16 payment was cashed on 1/**/26. This company sends me a bill every month for this co-payment. I called many times with no success. I wrote a letter on 3/**/16 included the...

payment from the bank with no success I am still receiving a statements that my balance is past due. I would like for this company to stop sending me a statements that my balance is due when I already paid this amount. Thank you.
