April 6, Revdex.com N 12th Street Phoenix, AZ RE: [redacted] Complaint To Whom It May Concern, Mr [redacted] is a former patient of the Glendale location of Arizona Urology Specialists; with office and lab visits in August and September of He was treated by DrPhilip K [redacted] for an initial office visitDrK [redacted] determined that a prostate biopsy needed to be doneThis was done on September 11, Pathology work was also done that dayMr [redacted] was also seen one additional time later in September of After sending all of his claims to insurance for processing; patient-due portions were determinedThese exceeded $ Patient did not make any attempt at restitution until a collection letter was sent to him in July, At that time the patient indicated he could afford to make $monthly installments on his balance His offer was accepted and payments were made routinely until late Mr [redacted] called in September, to ask for remaining balances to be adjusted off due to his not being happy with services providedThis is the first mention we have of this dissatisfaction documented from his callsPatient later indicated on another call that he would no longer make any more payments due to his dissatisfactionAUS already went beyond our policy of payment plan length in order to keep this patient in compliance with his obligations and away from Collection activities When patient failed to make a payment in December, his account was sent to outside collectionsThis was in the amount of $AUS has gone beyond what is required to allow Mr [redacted] to make payments on his balance which was allotted to him by his insurance company for services rendered Sincerely, Dennis LJ [redacted] Revenue Cycle Manager Arizona Urology Specialists, PLLC
Revdex.com: I have reviewed the response made by the business in reference to complaint ID [redacted] , and have determined that this does not resolve my complaint For your reference, details of the offer I reviewed appear below I have reviewed the response made by the business in reference to complaint ID [redacted] , and have determined that this does not resolve my complaintFor your reference, details of the offer I reviewed appear belowWe reject their response because it is almost the exact same response they gave to Department of Agriculture, Trade, and Consumer Protection complaint we filedKC Frescoln in his response to the Consumer Protection Investigator told them and in this response states, “When a medical practice “pre-authorizes” a code with an insurance company that a procedure is taking place it is not a guarantee that the code will be paid it is essentially a check to make sure that all the necessary steps leading up to the procedure have taken placeSo on that note had Judy even mentioned the word robotic to my insurance company during the pre authorization process the company would of said wait not all necessary steps have been met because no one mentioned robotics ever.oln also stated in his first response, " We are a small practice with limited resources and we do not verify every insurance plan and their associated benefitsIt is the patient responsibility to make sure they know their benefits when apprised of their treatment plan, which is the main reason that we have them sign the General Consent form." The Urology office attached a General Consent of which I signed on July to their response to the complaint under the information that the pre authorization was obtained correctly and all was approvedIt also says it is my responsibility if I choose to bill insurance it is my responsibility to ensure the rendered service is covered by my insurance carrierI was told it was pre authorized and covered for and a half months before I was operated onIf I cannot trust the doctor performing the surgery and his office personnel who can call in pre authorizations because I cannot call in my own pre authorization who can I trust? Jody whose job in the office is to pre-authorize procedures and surgeries admitted several times she called in the wrong codeIf this was my job to make sure it was pre-authorized why do they pay Jody? I am not able to call in my own pre-authorization, nor is anyone, lay person able to do that, it is the responsibility of the provider to do thatWhy I have attached support documents of other companies that do not hold the member liable for mistakes made during pre-auth by the providerRegards, [redacted] *chroeder
