I have reviewed the response made by the business in reference to complaint ID# ***, and have determined that my complaint has NOT been resolved because:
I saw other people at USATC after *** left were young men and was a young ladyThe reason for the changes is that I did complaint about each one and I sent USATC a letter To Whom It May Concern, Forover years USATC has been my Physical Therapist and I thoroughly enjoyedvisiting your facility, I told all of my family, friends and co-workers aboutyou, just to name a few individuals that I sent *** ***, *** *** and ***
***Your service was beyondexceptional and the staff was always professional and caring. As hard as PT was I looked forward to going ,even when my knee was sore I would go and leave feeling great, fast forward toAugust I returned to USATC after a*** surgery on the *** *** and started with *** *** this man workedmagic helping me to heal and to achieve full range of motion and then forwhatever reason *** left USATC, as a loyal customer I stayed and what amistake the service went fromsuperior to a grade so bad that I couldno longer continue here are some examples:1. The instructor would send me over to leg machineand tell me to do sets of and would not even come and watch my form orcount to make sure I completed the full set so the instructor used this time toconverse with other USATC employees.2. The stretch was so bad that I did not feelanything.3. I would arrive promptly at 5:30am and my first assignmentwas to do minutes on the stationary bike with no resistance and on SEVERAL occasions I would complete the minutes and then haveto wait another minutes before the instructor showed up.4. Then I was bounced around to new interns. I explained tosomeone there at USATC in accountingthat the service was horrible and I had no intentions of continuing with USATCor paying the last few co-payments and so now I am faced with a bill collector named ***
*** that called me on 3-**-at 12:48pm at which point I explained to him indetail what happened and then he calls me again on 3-**-at 12:02pm and whenI asked for the name of the company hebecame very abusive which violates the Fair Debt Collection Practices Act, 15USC 1692gIf and when I do get the name of the company litigation will be initiatedagainst themI am prepared to fight this injustice to the very end and utilizeevery city, state and federal agency possible.Thank You,*** ***PS-Copies of this letter are ready to be sent to thefollowing:*

after years of loyal serviceHe himself was a student hereHe left and opened up his own practice which is very successfulWe are all very proud of him
We have been sending *** *** periodic bills and calling to collect $of unpaid co-paymentsAfter years of trying to collect this relatively small fee we sent him to a collection company and he proceeded to report us to the Revdex.com
To Note: The patient never complained while he was here getting therapyHe is disputing co-payments for dates of service all of which were with ***He did receive full physical therapy treatments during the dates of service in questionThe amount of money owed is the responsibility of the patient as per the terms of his benefits with his insurance coverageWe are a contracted provider with his insurance
At this point the $doesn't mean much to me but his reporting of us to the Revdex.com is a huge concern for us

To Whom It May Concern,

For over 10 years USATC has been my Physical Therapist and I thoroughly enjoyed visiting your facility, I told all of my family, friends and co-workers about you, just to name a few individuals that I sent [redacted] and [redacted].

Your service was beyond exceptional and the staff was always professional and caring. As hard as PT was I looked forward to going , even when my knee was sore I would go and leave feeling great, fast forward to August 2012 I returned to USATC after a [redacted] surgery on the [redacted] and started with [redacted] this man worked magic helping me to heal and to achieve full range of motion and then for whatever reason [redacted] left USATC, as a loyal customer I stayed and what a mistake the service went from superior to a grade so bad that I could no longer continue here are some examples:

1. The instructor would send me over to leg machine and tell me to do 3 sets of 10 and would not even come and watch my form or count to make sure I completed the full set so the instructor used this time to converse with other USATC employees.

2. The stretch was so bad that I did not feel anything.

3. I would arrive promptly at 5:30am and my first assignment was to do 15 minutes on the stationary bike with no resistance and on SEVERAL occasions I would complete the 15 minutes and then have to wait another 15 minutes before the instructor showed up.

4. Then I was bounced around to new interns.

I explained to someone there at USATC in accounting that the service was horrible and I had no intentions of continuing with USATC or paying the last few co-payments and so now I am faced with a bill collector named [redacted] that called me on 3-**-14 at 12:48pm at which point I explained to him in detail what happened and then he calls me again on 3-**-14 at 12:02pm and when I asked for the name of the company he became very abusive which violates the Fair Debt Collection Practices Act, 15 USC 1692g. If and when I do get the name of the company litigation will be initiated against them. I am prepared to fight this injustice to the very end and utilize every city, state and federal agency possible.

[redacted] came to U.S. Athletic Training Center and we have no record of him having seen anyone other than [redacted] for physical therapy. The following is a quote from him: "for whatever reason [redacted] left USATC, as a loyal customer I stayed." [redacted] left U.S. Athletic Training Center after 7 years of loyal service. He himself was a student here. He left and opened up his own practice which is very successful. We are all very proud of him.

We have been sending [redacted] periodic bills and calling to collect $100 of unpaid co-payments. After 2 years of trying to collect this relatively small fee we sent him to a collection company and he proceeded to report us to the Revdex.com.

To Note: The patient never complained while he was here getting therapy. He is disputing 4 co-payments for 4 dates of service all of which were with [redacted]. He did receive full physical therapy treatments during the dates of service in question. The amount of money owed is the responsibility of the patient as per the terms of his benefits with his insurance coverage. We are a contracted provider with his insurance.

At this point the $100 doesn't mean much to me but his reporting of us to the Revdex.com is a huge concern for us.
