Customer requested US Network to act on their behalf and disconnect 2 lines that were no longer needed after they migrated their Centrex service to an [redacted] T-1. US Network submitted the disconnect request to [redacted] on January 12, 2012, Disconnect...

Order Number, [redacted] did not disconnect the service thus the billing for these two lines continued. Our investigation revealed that the services were disconnected in October 2014. US Network submitted to [redacted] an credit request dating back to January 2012. Claim Number established, [redacted]. The formula used to calculate the credit request were as follows; monthly charge of those two lines of $135.45 * 33 months. Total of $4469.85.
[redacted] received the credit request and offered the customer $2,234.93, the customer did not respond to [redacted]'s offer in the requested time frame and [redacted] cancelled the claim. US Network was directed by the customer to re-issue the claim again. The customer also found two other lines and a DSL circuit that we not disconnected dating back to 2012 as well, they asked us to go after additional credits for those services too. This credit the customer is requesting would be near $15,000. [redacted] has a Statue of Limitations, and will not likely honor the credit request. This account has been assigned to the Office of the President for review.
