Mr. [redacted] did sign up for an account back in April 10 of 2015. He signed up for an account that had a $14 monthly fee. This fee normally applies whether or not the customer chooses to receive mail or not as it is a rental fee, not an usage fee.Regarding the form 1583, the form is NOT required to...

activate the account. It is required to be able to ship items from the account. This is made clear in the welcome email. We apologize if the information was not clear or he got confusing information about this.Mr. [redacted] did request cancellation of the account in late September of 2015.Because Mr. [redacted] did request cancellation in late September and because he alleges that he was given incorrect information about the form 1583 we have contacted him directly and offered a refund of the fees charged as full settlement for this or any complaints Mr. [redacted] may have had.

The c**tomer is correct one of our staff members did make a mistake with the shipment. We did inform the c**tomer about the mistake and he requested a replacement letter which we forwarded a few days later in December 2015.We should have refunded the mistaken shipment. We apologize that we did not...

do that but the c**tomer had not contacted ** since the replacement letter was sent.We will now proceed with the refund of $29.48 as requested. This should settle any claims.We appreciate the c**tomer, he has been a long time c**tomer.
