I am responding to the complaint letter from John O[redacted] ID #10690239. Per the membership agreement Mr. O[redacted] would need to either send a certified letter or sign a cancel form at the club. Mr. O[redacted] did neither therefore his agreement was not cancelled. Mr. O[redacted] called on June 27, 2015 and spoke...

with Ashley to cancel. Ashley informed our manager on Monday and the necessary paperwork was filled out. Mr. O[redacted] requested that he would like a copy left at the front desk to pick up for his records.
