For clarification, Mr. [redacted]'s complaint is actually concerning his wife's billings. Per Mr. [redacted]'s request, I have sent via registered mail, on September 21, 2016 the original letter, a copy of which is attached to this response. As requested, I have included a copy of the EOB that we...

received from Blue Cross Blue Shield (BCBS) for payments received on his wife's most recent charge. As indicated in the attached letter, he should have received a similar document directly from BCBS, sometime in mid-April, which would have similarly outlined his portion of the bill that he would have been responsible for. I apologize to Mr. and Mrs. [redacted] for the billing challenges that we have faced. We have recently changed our billing vendor to improve this situation. However, in this particular case, we did bill the patient's insurance for all services rendered. The [redacted]'s benefit plan w/ BCBS left balances owing as patient responsibility.

I have investigated this complaint and identified the following:  1) The patient had provided and made payment on the outstanding balance to the billing company directly.  This information was not timely provided to the outside contractor Utah Imaging Associates engages to send pre-collect...

letters to patients.  Therefore the patient's account was still reflected as outstanding.  The patient subsequently received another notice and had paid it.  I have authorized a refund for the second payment.   2) I call the patient's father who had submitted this complaint and explained what had happened.  I assured him that his daughter's credit had not been negatively affected as no outside credit agency had been notified.  I thanked the patient's father for bringing this to our attention as we were able to identify the billing company's untimely transfer of information to the outside collection service.  I believe the problem was resolved to the complainant's satisfaction.   Patricia H[redacted], CEO

I have reviewed this account and found the following information:As is common practice in radiology billing, the patient will receive a bill from the facility for what is commonly referred to as the technical charge, and a bill from the Radiologist, commonly referred to the professional charge. this instance we had not been paid for the professional charge on either of these exams; however, the technical charge had been paid. Indeed the claim that we sent was actually denied. We have since contacted [redacted] to figure out why our claim was not paid both claims were first rejected as not covered by [redacted], then they were rejected as Patient could not be identified. Both of these rejections were errors. As of today, we have faxed these claims over to [redacted] for processing and adjudication.
