Patient came to see us on 12/after falling while snowboarding She was concurrently seeing us for an auto injury as well The visits dated 12/28, 01/02, 01/04, 01/08, 01/10, 01/were all submitted and DENIED by the auto case They denied all of those visits based on the fact that she had a snowboard fall and they contributed all of these services to fall under a commercial insurance instead We submitted all services to her commercial insurance - Physical Therapy visits from 01/02-01/were all paid by her commercial insurance company - [redacted] *** The patient did NOT have chiropractic coverage with this insurance policy She had a punchcard that was purchased for her chiropractic care That punchcard had visit remaining on it We used the remainder of the punchcard for her 12/and 01/visit She was seen additional times 01/04-01/with no punchcard available The next punchcard was instated to cover those visits1,2,& of visits were used for those remaining dates of service Patient currently has punches remaining for her use on those punchcards The purpose of a punchcard is to discount services, so patients don't have to pay out of pocket a large amount, instead they purchase a punchcard and all services are covered at a discounted price The cost of a punchcard is $for visits which equates to approx $68/visit Actual charges for ** [redacted] for those non-covered visits were $of which we wrote off anything above and beyond the $ If ** [redacted] doesn't want to have the remaining punches available for her to use, we would discount the visits without a punchcard discount (that would be visits on new punchcard) as follows $per visit + $for re-exam for a new injury, that total would be $and we would refund her the $125.Please let me know what ** [redacted] would like to do Please note that we have never been contact by ** [redacted] regarding this issue and this is the first we are hearing of this We would have gladly have gone over this information with her as we have here One simple phone call to the office would have avoided this problem[redacted] - Office Manger - VIMG -

that she had a snowboard fall and they contributed all of these services to fall under a commercial insurance instead.  We submitted all services to her commercial insurance - Physical Therapy visits from 01/02-01/21 were all paid by her commercial insurance company - [redacted].  The patient did NOT have chiropractic coverage with this insurance policy.  She had a punchcard that was purchased for her chiropractic care.  That punchcard had 2 visit remaining on it.  We used the remainder of the punchcard for her 12/28 and 01/02 visit.  She was seen 4 additional times 01/04-01/21 with no punchcard  available.  The next punchcard was instated to cover those visits. 1,2,3 & 4 of 7 visits were used for those remaining dates of service.  Patient currently has 3 punches remaining for her use on those punchcards.  The purpose of a punchcard is to discount services, so patients don't have to pay out of pocket a large amount, instead they purchase a punchcard and all services are covered at a discounted price.  The cost of a punchcard is $475 for 7 visits which equates to approx $68/visit.  Actual charges for **. [redacted] for those non-covered visits were $957.00 of which we wrote off anything above and beyond the $68.  If **. [redacted] doesn't want to have the 3 remaining punches available for her to use, we would discount the visits without a punchcard discount (that would be 4 visits on new punchcard) as follows $75 per visit + $50 for re-exam for a new injury, that total would be $350.00 and we would refund her the $125.Please let me know what **. [redacted] would like to do.  Please note that we have never been contact by **. [redacted] regarding this issue and this is the first we are hearing of this.  We would have gladly have gone over this information with her as we have here.  One simple phone call to the office would have avoided this problem.[redacted] - Office Manger - VIMG - 970.926.4600
