To whom it may concern,
Regarding complaint ID [redacted] Valley Vision and Hearing Associates will not be removing the charges of $63.75 for the contact lens training. It is required by Washington State Law that a patient who desires to wear contact lenses have a training by a Certified Contact...

Lens Technician. [redacted] signed a policy stating that he understands the terms regarding the charges that are associated with this training and the contact lens prescription. After the training which was completed on 5/11/16 we do a follow up with the Doctor or Technician to evaluate the fit, vision, and comfort on 5/18/16. After that we will do phone follow ups. [redacted] was contacted three times following the visit with Dr. D[redacted] 5/23/16, 5/31/16, 6/6/16. At this time [redacted] was “trialing” 2 different kinds of lenses. The patient did not respond to our attempt to “finalize” his contact lens prescription. After the 4th week we billed the training only because the prescription was not finalized as he never responded.  It is our policy to NOT bill insurance unless specifically requested by the patient. After a conversation with his insurance VSP-Jenny on 6/23/16 regarding his benefits they suggested to the patient that he use his benefit for glasses. [redacted] has never requested that we bill his insurance. Valley Vision would be happy to bill this service to his insurance at his request. If [redacted] would like his contact lens prescription we would be happy to write that as well but we have to know which lenses are comfortable for him. Being aware that there will be a separate charge for the prescription itself $29.75. This charge can also be billed to his insurance VSP at his request. Several attempts since then with phone calls, emails, and facebook messages patient has been non-compliant, therefore not all services have been completed hence not having a contact lens prescription written.
