As stated earlier, I contacted the VA office first, a form was sent to me via email form Santiago G[redacted].  I could not print the customer complaint form that needed to be signed and sent back.  Tried contacting him numerous times, no response.  Called DC branch last week for...

assistance in getting the form and explaining the situation and was  practically brushed off.  The man insisted that I must deal with the VA office, so after numerous complaints to different offices, Mr. H[redacted] from VA reached out and sent doc.  I sent him what was needed, after reviewing, I was told that I had to do this in DC.  Maybe if the gentleman from DC had listened more closely than I would not have gone through the run around.
