TO RESOLVE THE ISSUE THE COMPANY WILL NEED TO STOP USING SOCIAL SECURITY NUMBERS IN THE BILLING PROCESS

We have changed our policy of billing so this will not happen again as stated in the letter It is a requirement of most employer to have the social security number listed on the bill for the Occupational Health ServicesThe social security number is an identifier so when we send the bill to the employer requesting the service they know which employee received the service and who they are paying the bill on This was not an open disclosure of [redacted] social security number [redacted] employer has a contract with our clinic for confidential services and Gale provided his social security number in the past on the Company account and his social security number was linked as an identifier on that account Per our conversation with Gale we have removed his social security number from this account His company requested the service and we bill the employer per our contract In our conversation with Gale he stated he works at the company and opened the billing statement we had sent to them Thank you, [redacted]

In good faith, DrJohnson, a Family Practice physician, referred Ms [redacted] to DrThomas, an Ear, Nose & Throat specialist for her condition He did so in order to help Ms [redacted] address the symptoms she was experiencing The charges associated with her seeing DrThomas totaled $and the balance due after insurance was $ Ms [redacted] also owes our clinic for visits on January 21, for $126.59, February 3, for $52.14, February 10, $and February 14, for $with various Health Care ProvidersOur Operations Manager contacted Ms [redacted] and [redacted] , M.D., our Medical Director reviewed Ms [redacted] ’s concern and sent her a letter We also attempted to correspond with Ms [redacted] using the address we had on file for her which was [redacted] ** [redacted] ** *** I note from her correspondence to Revdex.com that her address now is 11th Street, Hudson, WI I also had left messages for her, but am uncertain that they went through as no personal greeting was on the voice messageAs a show of good faith, we are willing to provide a courtesy discount of 20% off her bill with DrThomas of $if she makes payment in full by November 30, No further discount will be offered, as the referral was appropriate given her symptoms and condition at the time of referral Additionally, we do not understand why Ms [redacted] hasn’t paid the bills for the additional dates of service We are not willing to discount the bills for those servicesThanks, [redacted] , Administrator

Patient contacted Vibrant Health Family Clinic's on May 22,regarding a statement that was sent to his EmployerThe service was requested by his employer and we hold an Occupational
Health Contract for these servicesSocial Security numbers are required on chain
of custody forms as a source of identification because this is a confidential service per his employerPatient did not want to give his social security number at his April 29, visit and gave the lab tech his driver license numberIn past years patient had given us his social
security number for this test and we had it on his accountOur computer software vendor has a standard claim form for Occupational Health that uses the social security number as an identifierOn May 27, I contacted our computer software vendor to see if we could remove the social security number from the invoiceThey have found another form we can use that does not list the social security number and we have already put this process in placeOn May 29, we called patient and explained to him why his social security number was on the form and that we have put a process in place to use a different form so social security numbers will not print out on the invoiceWe told patient that we would send out a corrected

DrJohnson, a Family Practice physician, referred Ms*** to DrThomas, an Ear, Nose and Throat specialist for her condition on December 2, 2013. On January 3, Ms*** had an appointment with DrThomas. January 8, Ms*** called in to our Operations Manager with her concerns about her visit with DrThomas and she notified Ms*** of DrJohnson’s retirement. The Operations Manager submitted Ms***’s concern to our Medical Director who thoroughly investigated this using our peer review process which included chart review, and discussing the case with DrJohnson and DrThomas. DrJohnson also called Ms*** on March 20, who explained why he had referred her to DrThomas and this was appropriate. Our Medical Director sent a letter to Ms*** explaining that the clinic had investigated her concern and the care she received was found to meet the standards and the charge will stand.

When I read the medical notes, I was so surprised to see what he put in them. He never mentioned the acid reflux as a possible issue or an ENT specialist. Further, Dr. Johnson typically dictated his notes from my previous visits so I could hear the summary of what had been discussed. On that visit in December, he did not do so. When we discussed this in March, Dr. Johnson asked me what I expected and I said that I expected to not be charged for the visit to Dr. Thomas as we did not discuss an ENT specialist, rather, we discussed ruling out possible asthma as to my breathing issues and having a spirometer test done.
