[redacted] , you stated the office left messages about waiving the fee, I never received any messages nor did I ask you all to waive any prior broken appointment feesI was not aware of the $policy as of July 8th Also you guys cancelled on me when the dr had the baby the same day so why was that ok? [redacted] you wont even contact me back like I requested, this is the first time I've heard from youI disagree with your statement saying this could easily been resolved because you have not contacted me on this matter priorI still believe that your policies were not done properly because I never received any messages from your office in regards to any fees being waived, you guys made that decision on your own without properly notifying meVerbal or Written was not given to me about that decisionTherefore I should be allowed to ask for a waiver based on that information not allowing me that choice to be made

our office policy $20.00 charge for an appointment not broken within 24 hours.
Our office will waive the first broken appointment fee and advise the patient how to avoid the charge. [redacted] was advised of our policy on July 8th, 2015. She had called the office 45 min prior to her dental appointment and rescheduled to September 10, 2015.
The office spoke with [redacted] on October 14, 2016 due to our doctor out of the office for family matters. She rescheduled to October 22, 2015. Later that day she called to cancel her appointment and did not reschedule.
December 1, 2015 [redacted] no showed her appointment. We left a message to reschedule and informed [redacted] that the fee was waived but there would be a charge next broken appointment.

I was not aware of the $20 policy as of July 8th 2015. Also you guys cancelled on me when the dr had the baby the same day so why was that ok? [redacted] you wont even contact me back like I requested, this is the first time I've heard from you. I disagree with your false statement saying this could easily been resolved because you have not contacted me on this matter prior. Verbal or Written was not given to me about that decision. Therefore I should be allowed to ask for a waiver based on that information not allowing me that choice to be made.
