I am in receipt of the complaint from this clientI am attaching an e-mail I received from her confirming receipt of the replacement documents on October 31, I hope that resolves the issueAs to the underlying complaint of misspelling the name of the deceased, while it is true that the client provided us with a couple of identification documents, for the purposes of data entry we rely primarily on a “vital statistics worksheet" that we ask our clients to fill outThe original death certificate lists the name of the deceased as it was spelled on the worksheetThe worksheet also provides an area for adding an alias or "aka"None was addedAmong the documents provided by the client and that we received from the hospital, are the deceased's social security card, medical records, and checks provided by the client for payment of our servicesAll of these documents have the deceased's name spelled as we had it on the original death certificateFurthermore, the instructions for filling out the death certificate from the California Bureau of Vital Statistics state the following: In some cases, the decedent may have more than one “legal" nameThis most often occurs with individuals born in another countryThe name
to be entered in Fields through is the name that was used to verify the Social Security NumberIf there was no Social Security Number verification, then use the name used for official purposes in the United States, for example for a drivers license, tax purposes, voter registration, etcIf there is another legal name, it should be placed in the AKA (Also
Known As) fieldsUnfortunately, the fact that the deceased spelled his name in two different fashions was not brought to our attention until after the death certificates had been filed and printedThis situation could have been avoided if we had made a stronger effort to clarify the spelling during our initial meeting with the clientWe understand that our clients have many thoughts and emotions running through their heads when they come to our officeGoing forward, we will work to draw out information that is as clear and complete as possible
