Dr. L provided exceptional service and treatment. My complaint is with the way billing has been handled. I went through a divorce 2 yrs ago and was still on my husband's insurance at time of service. His insurance company erroneously ended my coverage one month before it was supposed to, and at first denied the claim. Dr. L's acct person notified me that the insurance did not pay. I talked to my ex husband, who spoke with his insurance admin and had the insurance reinstated for the date of service. I thought that Dr. L's office would refile the claim, but they did not. I had seen Dr. L several years earlier when living at my prior married address, but when I saw her in Jan 2016, I gave them my new address and asked them to update it. They did not update my address and sent all bills and collection notices to my old address, where I had not lived for over a year. Because I had not received any communication from them at my new address, I assumed that the account had been refiled with the insurance and paid. I then saw on Credit Karma in Aug 2016 that the account had been placed in collections. I called them back and asked why I had not received any notices of the bill or collections, and they said they had mailed several. When I asked what address, they gave me the old one. I asked them to update the address and refile with my insurance. They did, and I received a letter from them in Nov 2016 stating that insurance had paid the claim, and they had written the credit bureaus to have it removed. I called back in May 2017 to say that it had still not been removed from Equifax and asked them to send another letter and they said to send the one they had sent me. I filed a dispute with Equifax who said that *** showed no pmt on the acct as of Sep 2017. Dr. L's office received pmt from the insurance co., but did not notify the collections agency that they had been paid and apparently did not pay the collection company what they owed them.

Desired Outcome

Correction to a credit report I would like for Dr. Lohse's office to contact *** and pay them what they owe them to have the account removed from my name. I would like for them to contact Equifax with an explanation to have it removed from my credit report. I would like copies of the correspondence.

Patient received services in our office 1/22/16. We agreed in good faith to file insurance for payment on patients behalf. Insurance was denied due to cancellation and EOB was received in our office 3/8/16. Insurance was appealed and decision remained denied. Patient was billed for balance 4/28/16, 6/1/16 and 7/6/16 as well as collection calls made on these dates. No contact was made with our office by patient in response to phone calls or statements. Patient also received correspondence from insurance company notifying patient of denial of services. Account was placed with Capital Accounts collection company due to lack of communication from patient on 8/3/16.
We received a phone call from patient in October 2016 stating that insurance claim was incorrectly processed due to her divorce and that she did not receive any correspondence from our office. Patient stated that she had moved and thought that she had given our office an updated address however we had no corrected contact information from the patient in her records. In good faith on this patients behalf, we again reached out to the insurance company, submitted letters and documentation to get this claim paid for the patient. After re-processing several times, we were successful in getting this claim paid. A letter was immediately submitted to capital accounts to remove this patient from collections. A transmission was received confirming the cancelling of this account from Capital Accounts.
Our financial policy, signed by patient, states that we will assist in making collections from insurance companies however if insurance company has not paid within 45 days, full payment is required from the guarantor immediately. See all attached documents.

I appreciate the response and attention to this matter and the supporting documents from Dr Lohse's office. However, there are some things that I do not agree with. As you can see on the first page of documents, I crossed out my old address (5516 Gallatin, where I had not lived for over a year) and wrote in the new address (3085 Park West). When I called them in Oct 2016, the address they gave me on my acct was Gallatin. They had not updated my file with the new address. Therefore, I never received any statements since they were sent to my old address. I also never received anything from the insurance and never received any phone calls from the Doctor or from the Collections or any email or any postal mail. The only way that I found out about it was seeing it on my credit report on Credit Karma. I called Dr. Lohse's office as soon as I realized that it had not been paid and to ask for it to be refiled with insurance, which they did, and insurance subsequently paid. I just spoke with Capital Accounts again and it is scheduled to be removed from my account and credit report in Dec. I appreciate Revdex.com's assistance in helping to get this matter resolved, and Dr. Lohse's office for their response.
