Review: I call to make an appointment at cefola Vincent and roger chehovs dental office before I confirm the appointment I told the receptionist that my insurance carrier is 1199 I the ask her if my insurance was accepted there I was told yes my insurance was accepted there, I confirm the appointment.the went to my schedule appointment, before I left there they wanted to know if I have a secondary insurance, I ask what is the reason for asking for a second insurance I am the primary on my insurance, I was told it, s just in case , I gave them my husband insurance card, they bill my insurance then turn around and try to bill my husband insurance, when they were not able to obtain double payment, that was when the harassment started, I have contacted my insurance carrier and I was told by my insurance that payment was made to cefola vincent and roger chehovs.so I do not owe them any money so they need to stop with the harassment.Desired Settlement: I want the harassing phone calls to stop and the collection to stopbecause I do not owe this dentist any money, beside harassing me and sending my name to collection is pure greed on there part and I knowthey are trying to get double paid, they are trying to scam me and the insurance company and they need to be stop.

In response to complaint about billing issues with [redacted], these are the details of the matter.

[redacted] presented to our office on 01/**/13 for a consultation for a "loose front tooth ". A periapical x-ray was taken and a limited oral examination was performed. The patient was advised on the treatment necessary. On 02/**/13 she

presented again as an emergency with the complaint "crown came out". The crown on her front tooth was clean d and was recemented back on .

[redacted] as two insurance policies. A primary and a secondary insurance. The primary insurance is 1199 Union and the secondary insurance is Cigna Dental. The claim was submitted and payments were credited to her account. Total treatment totaled

$225.00. (See enclosed patient ledger). After payment from both primary and secondary insurance carrier s there was a balance of $48.00. [redacted] was billed this balance but has refused to pay. The bill was t hen turned over to a collect ion agency.

I do not participate with either 1199 Union or Cigna Dental Insurance plans.

Therefore, t he balance is due from the patient and we are required by law to bill this balance to the patient. [redacted] is under the impression that just because she has two insurance plans that she does not have to pay any remaining unpaid balance. I do not accept either insurance as full payment so she is liable for the $48.00 balance.

[redacted] is the guarantor for her two children [redacted] and [redacted].

They too presented m y office. [redacted] was treated with an intraoral complete set of x­rays and a cleaning (prophylaxis) on 02/**/ 13. The total treatment for [redacted] was $224.00 (see en closed patient ledger). The 1199 Union insurance paid $50.00 on the claim . The secondary insurance Cigna did not pay anything on this claim due to an age limitation . Copy of the claim form is en closed with a clearly marked message stating that the member is required to make the copayment (balance) of $174.00.

On 02/**/13 [redacted] came in for treatment in my office. Four bitewing x -rays and two periapical x-rays were taken in addition to a prophylaxis (cleaning). On 02/**/13 [redacted] had a comprehensive exam and restoration s were placed on teeth # 12, 13 and 15. Tooth # 19 was also extracted on this day. The treatment total came to $ 1078.00. (sec enclosed patient ledger) . I received a payment of $38.00 an d $ 152.00 from 11 99 Union insurance. The secondary insurance paid zero, again, due to an age limitation on the policy. (they are only covered till 19 years old). The Cigna policy for both [redacted] and [redacted] had been terminated at age 19. As clearly stated on the 1199 explanation of benefits enclosed the member is responsible for the co-payment (balance of $888.00 on [redacted]) .

As clearly stated on the enclosed ledgers and benefit statements, [redacted] is responsible as guarantor for a total of $ 1 ,100.00 for herself and two children. If you have any question or concerns, please call me to discuss. Thank you.
