On June 1, 2019. I went for an eye appointment to get new prescription glasses and contact lenses. Vista Optical has two different departments. So I needed to do the eye exam and order contact lenses on one side and order my prescription glasses in another. My insurance paid for most of the exam, and I only had to pay $38 out of pocket. I had to pay $70 for the contact lenses (total of $108.00). I then bought the eyeglasses and only had to pay $18.00 out of pocket. I received the eyeglasses in about a week. After about 2 months passed I never received the contact lenses. I called on August 2nd and talked to the clerk and she informed me she could not find me in her system. She try different versions of my name, but cannot find me in her system. She kept asking me if I was sure I came there or maybe I went somewhere else. I made it quite clear this was the place where I got my exam and order my eye correction devices. I found my receipt along with my prescription information and gave her the numbers on the forms. She was finally able to find my information, minus my contact lenses prescription and order. She inform me that the contact lenses were never ordered and I hadn’t ordered any. I then inform her I have my bank statements informing the amount that was debited from my account. She did not have a copy of how much eye exam was or what was charged to me, only my eyeglasses order receipt. I order Acuve Moist contact lenses. This company has my money but has not provided the product. I want either the contact lenses or a refund.

The receipt from the doctor was sent a bit cloudy I tried to clean it up a bit with the scanner bit this was as good as I could get it. The itemization for the exam is as follows:
Exam $240
Refraction $40
Contact lens fitting and evaluation: $80
Elective retinal photography: $100
***'s copay was 10% $28
She was also charged for the $80 contract lens fitting and evaluation.
***'s total out of pocket for the exam alone was $108. One set of trial lenses are often provided when applicable. The remaining balance was billed to her insurance carrier. This is an estimation of covered amounts based on the eligibility call with the carrier. Any unpaid balance or overpayment by the carrier may result in a refund or invoice being sent out.
If *** would like an unredacted copy of her receipt she may stop in and pick one up or provide a medical release and they can send one over if she would like.
The eyewear are as follows:
Right Lens single vision poly $41
Right lens premium anti reflective coating $35
Left lens single vision poly $41
Left lens premium anti reflective coating $35
Frame $59
Total charges: $211
Insurance carrier was billed $211 for these services.
*** elected to purchase a product protection plan at an additional cost of $18. This was not billed to the carr6as it is not a covered item. ***'s total out of pocket for the eyewear was $18. We will submit a claim to the insurance carrier and and underpayments or uncovered items may result in a refund or invoice being sent out.
