I am rejecting this response because: X-rays was done was my initial appointment to the cleaning I felt the x-rays were unnecessary Healing to what? Healing of the implant process was later, not the initial appointment.I stand to my original complaint this dentistry is in for charging unnecessary fees and did poor dental work to recoup expenses for his newly furnished dentistry practice in a nicer new building

We have already provided all the information we have between us and Mrs***In regards to rejecting x-rays from another office we inform patients that we will accept the x-rays but, they must be current otherwise we would require new x-rays especially when it comes to implants as we need to see how the healing is going as well as the bone integrationWe feel we have provided Mrs [redacted] with the best service as possibleIf you require any additional information please contact our office

I am rejecting this response because:On my first initial visit 5/12/11, I found out *** did not want to take x-ray records from other dentist offices Other dentist offices that I went, they all take transfer x-ray records from the old dentist offices *** insist on wanting their own x-ray records once they found out my insurance hasn’t been billed and is valid for new x-ray even though I insist to have x-ray records transfer instead. Providing identification as *** calls it versus having the Dentist charging insurance for x-ray are two total different points X-ray was not needed as the transfer records would be good enough yet *** insist on charging. The additional waiting time was after all the new patient form was filled out. The following day *** staff called to do a survey and I gave my honest answer about the additional wait time. First attempt of crown was in the shape of a triangle Natural teeth does not come in triangle shapes Crown should imitate a natural teeth shape, not a triangle The triangle would leave two extra side spaces between my crown and the two surrounding teeth so I told the Dentist *** that is not going to work. Please note in ***’s description of 7/7/office visit where Dr*** says, “The crown was temporarily cemented as we prefer to do for the implant crowns.” I will come back to this statement later. Background information on the late and early time period was when the dispute was filed with Delta Insurance and therefore Patient request records were requested to be sent. When crown came off in less than years, I went back to have it cemented Dentist still blame it on the “small” size implant/abutment, the work of another office To cement it permantly? I was not given nor told of this option Please see the 7/7/office visist where choice of Dr***’s option was to cement temporarily. So on 4/6/appointment, I realized Dentist wanted to do an Emergency Service charge and a new x-ray record charge This all brings back to the point this Dentistry wants to charge to the max. The other point about the teeth whiten and teeth straighten services The hygienist recommended me to do the ‘Smile’ invisible brace where I didn’t ask about that The office nor hygienist never offered me a free whiten kit. I certainly have maintained my teeth care to the best as the Capitol Periodontal office that I go too has said that many time No cavity or other teeth problem since 2011-present except for the poor crown work of *** that keeps on coming off

On 5/12/patient came in for a new patient examination appointment, we require a form of identification from every patient and patient did not want to provide us with her identification at firstWe had to repeat our office policy several times before patient provided us with itThe following day
we did a follow up call to check how patients visit went and patient complained about waiting min from her scheduled appointment time to being seatedBeing called back 20mins after appointment time had to do with the time it took patient to fill out new patient forms and provided us with the required insurance card and identificationPatient came in with an implant that was placed from another officeWe informed patient that the implant was really small to carry a molar crown but, on 6/23/we attempted to do a cerec implant crown, but patient did not like it so impression was taken for the crown to be sent to the labThe crown was cemented as a temporary until we received the permanent one from the labOn 7/7/patient came in for the crown delivery but, patient was still not happy with the crown, patient has very high expectations that we were not able to meet so we informed patient we would only charge her for the lab & material fees and we refunded the rest of $so she could go elsewhereThe crown was temporally cemented as we prefer to do for the implant crownsPatient called us in September and scheduled appointment for 9/18/at 2pm for crown that broke, but then patient called same day of appointment to cancel indicating the 2pm appointment is not a good time for her, we tried to reschedule appointment but, patient declined saying not at this time9/23/patient called and requested x-rays to be sent to another office, so we emailed x-rays to another office10/9/patient requested us to email x-rays to her directly, so we emailed her x-raysOn 3/24/we received a grievance patient filed with insurance company for the implant crown that broke 2yrs after it was completedWe sent all requested documents to insurance company and on 4/11/we received a response back from the insurance company denying the grievance patient had filed agreeing that the services we provided were performed correctly4/26/patient came in for appointment for a broken implant crown and we informed her we would do a new implant crown for the same fee she was refunded $and no additional fees would be charged as a courtesy5/5/patient wanted to cancel her crown delivery appointment that was scheduled for 5/10/as she was hesitant about using our office and since she received a letter from her insurance company indicating she can go to any other office and they would allow payment for an implant crown replacementWe informed patient of lab fees that would apply if she were to cancel as the crown was already completed, but patient decided to keep appointment for implant crown deliveryWe delivered crown on 5/10/and used temporary cement1/15/crown came offInformed patient crown would continue to come off because of the small size of the implant and abutment and in her case it would be better to cement it permanently, but patient refused to permanently cement crown so re-cemented crown with temporary cement, office visit was billed to insurance but, we did not charge for the re-cementation4/6/patient came back with crown off, informed patient it would be best to cement it permanently and patient agreed but, refused to pay for anythingInformed patient since it’s been years since crown was delivered and cemented with temporary cement at her request, we would not proceed with services if no payment was madePatient insisted not to pay anything and said she was going to file a complaint so we refused the service due to her history of being difficult with our office policies, patient has been difficult since she first came in and we tried our best to accommodate her many times beforeWe did not charge patient for the office visitPatient also indicates we told her she needed whitening and braces, we never tell patients whitening is needed unless patient is interested and then we offer a FREE whitening home kit for all of our patientsWe are dedicated in providing the best care for our patients that is why we will repair or replace a crown restoration within a year periodTo keep this coverage valid, we ask to follow certain guidelines: 1: Maintaining continuing care (months) 2: Maintain proper home care 3: Complete recommended treatment Since the patient did not comply with the mentioned guidelines, we do not feel it is fair to refund the crown or continue to provide services at no chargeWe went over and beyond to meet her expectations

We sent all requested documents to insurance company and on 4/11/14 we received a response back from the insurance company denying the grievance patient had filed agreeing that the services we provided were performed correctly. 4/26/14 patient came in for appointment for a broken implant crown and we informed her we would do a new implant crown for the same fee she was refunded $824.80 and no additional fees would be charged as a courtesy. 5/5/14 patient wanted to cancel her crown delivery appointment that was scheduled for 5/10/14 as she was hesitant about using our office and since she received a letter from her insurance company indicating she can go to any other office and they would allow payment for an implant crown replacement. We informed patient of lab fees that would apply if she were to cancel as the crown was already completed, but patient decided to keep appointment for implant crown delivery. We delivered crown on 5/10/14 and used temporary cement. 1/15/15 crown came off. Informed patient crown would continue to come off because of the small size of the implant and abutment and in her case it would be better to cement it permanently, but patient refused to permanently cement crown so re-cemented crown with temporary cement, office visit was billed to insurance but, we did not charge for the re-cementation. 4/6/16 patient came back with crown off, informed patient it would be best to cement it permanently and patient agreed but, refused to pay for anything. Informed patient since it’s been 2 years since crown was delivered and cemented with temporary cement at her request, we would not proceed with services if no payment was made. Patient insisted not to pay anything and said she was going to file a complaint so we refused the service due to her history of being difficult with our office policies, patient has been difficult since she first came in and we tried our best to accommodate her many times before. We did not charge patient for the office visit. Patient also indicates we told her she needed whitening and braces, we never tell patients whitening is needed unless patient is interested and then we offer a FREE whitening home kit for all of our patients. We are dedicated in providing the best care for our patients that is why we will repair or replace a crown restoration within a 5 year period. To keep this coverage valid, we ask to follow certain guidelines: 1: Maintaining continuing care (6 months) 2: Maintain proper home care 3: Complete recommended treatment Since the patient did not comply with the mentioned guidelines, we do not feel it is fair to refund the crown or continue to provide services at no charge. We went over and beyond to meet her expectations.
