[redacted] Please See Attached Documents [redacted] I received your letter late last weekWe are so sorry that Mr [redacted] has experienced such atremendous loss and hope that the information we provide will be helpful.To provide a timely response, I am faxing my reply and following up with a mailing which will include thedocumentation necessary for Mr [redacted] to make the changes he requested Respectfully, Gary W***, Member/OwnerWallin Funeral Home & Cremation, LLC I am in receipt of you letter dated February 5, and am glad to respond to the complaint:.A young man I shall identify as [redacted] died at Harborview Medical Center on August 24, 2012.I was contracted by the family to provide funeral servicesI met with his mother whom I shall identify as **, toobtain vital statistics information for the Washington State Death CertificateOne of the sections on the deathcertificate asks for the name of the mother and father [redacted] provided me with her name and for the 'father's sectionshe provided me with the name of a person I shall identify as **.Prior to filing, [redacted] approved the proof copy of the death certificateThe death certificate was filed with thestate on August 30, and death certificates were obtained for the familyFuneral services for [redacted] were held onSeptember 14, [redacted] paid for the funeral services on September 14, 2012.On or about October 9, 2012, while we were conducting a funeral service, a gentleman entered our facilityclaiming to be the legal father of **I will identify him as **I had no knowledge of this man and his abrasiveactions, verbal demands, and odor of Intoxicants was very upsettingHe demanded I change the death certificate for [redacted] to show him as the fatherHe provided me with a photocopy of a document from the courts from and also aphotocopy of a complaint he had apparently made to the Island County Sheriff's Office which involved a personaldispute with [redacted] and other family members.I informed [redacted] that he would need to contact King County Health Department where the original deathcertificate was filed and provide them with official documentation about his claim of paternity and also file anAffidavit of Correction with the State of WashingtonI provided him with a blank Affidavit of Correction and showedhim how to apply for correctionHe would need to provide complete legal information to the state.I repeatedly Informed him I did not know who he was and did not know if his claims were validAll heshowed me were photocopies of a court ruling and a photocopy of a statement he had obtained from the IslandCounty Sheriff's OfficeI told him he would need original legal documentsHis verbal statements and oldphotocopies were not legal proof of facts[redacted] left abruptly and very angryI heard nothing from him until an e-mail sent from him on February 4, 2016.This was four years after our one and only encounter I did what I could to assist **I instructed him on how to goabout making the changes he requestedI honestly felt that [redacted] had followed my instructions and that the changeshad already been made, so I was very surprised to receive an email from him as well as your letterFuneral homesdo not get updates from the state about completed Affidavits of Correction so I had no way of knowing the changes [redacted] requested were not made.Our instructions again to [redacted] are to obtain certified copies of the court records naming him as the biologicalfather of **Those documents, along with a Washington State Affidavit of Correction should be mailed to the Stateof WashingtonI have included a partially completed Affidavit of Correction to assist him in filingWe have alsoincluded a mail in request form for certified copies that [redacted] can order from the state once the change is made.Please let me know if you require additional informationThank you.Respectfully,Gary W***, Member/Owner

Complaint: ***I am rejecting this response because:
First of all it is not an accurate response as to the facts or the events,and their actions did cause damages and hardship that need be addressed I feelThe business stated and assured me that if I brought a copy if the paternity documents to their establishment,which I did do,just to be "wrongly and falsely accused of being intoxicated"? I will have it known that I am allergic to alcohol so that is also not a possibility.I also have labs that show I was only taking prescribed medications at the time and that's for a form of cancerMr W*** had the opportunity and means to "Ammend the certificate" and maliciously refused and did so under a blanket of factsThis could have all been easily remedied had he not taken the inappropriate actionsThe statement of "falsely being intoxicated" were slanderous and I would like to know on what basis he based this? My being distroight with seeing my sons pictures all over his establishment perhaps ? The fact I had been crying due to this being my second son to have died and within a mile of each other? How about how someone had lied to be considered his father? I can produce witnesses and documents to show this is need be and I have already inquired about a polygraph as I have no reason to lie and find this quite appalling at best Sincerely,*** ***
