This patient has insurances, Medicare and VAWe have filed the claim with both and both have deniedHe has Medicare part A and VA coverage after medicareambulance services are not covered under part AThis needed to be billed to his VA insurance as Medicare denied on 4/26/We were
informed about his VA coverage on 6/3/and billed his VA insurance on 6/3/Well within daysThe problem is Va will not pay for an ambulance claim that they do not have a corresponding hospital bill forThe hospital portion of this patient was paid for under Medicare Part A, meaning they did not bill VA and VA will not pay his ambulance billThe patient does have the right to appeal the claim denial with his insurance company if he files it within a timely mannerI have attached the call comments to show our followup on the claim and our conversations with VA about denialAlso attached is the call historyThis shows we filed the claim timely with his insurance companiesMedicare was filed on 4/14/and denied 4/26/VA was filed on 6/3/( as noted by the "INSURANCE HCFA FORM") and denied 8/29/2016.If the patient has further questions about this bill being denied he should contact both of his insurance companies for clarificationDue to non payment by insurance this bill is patient responsibilityAs far as contacting ORMC for patient insurance information we do have contact with them, but again their portion of the claim would be paid by Medicare and they do not have VA coverage on fileAnd we sent the patient letters informing him of his insurance denials and stating the bill is his responsibility just as we do with any patient who has insurance that denies the claimThe patient is sent letters prior to a letter informing them that they may go to collections, the patient can contact us to set up payment arrangements so the claim does not go to collection, instead he sent letters stating he isn't going to pay and never contacted our office to set up a payment plan

I have reviewed the response made by the business in reference to complaint ID [redacted], and have determined that this does not resolve my complaint.  For your reference, details of the offer I reviewed appear below.There are a few things that I have a problem with, first the VA in the past has always informed me of any claims being rejected by office letter in the mail. I have yet to receive such letter. Second at no point did the company in question ever contact me regarding these issues other then send me a repeating bill. At this point I am still not going to pay this bill unit all issues are resolved whcih it seems I might have to contact the VA to see the issues at hand. Until then I am not accepting there answer nor am I making any payments as in hte past the VA has made payments once all the correct information had been provided. I am not sure why Part B was in the picyture and why the company assumed I had that coverage. Things are just not adding up and like history show the veteran is the one holding the bag with little assistance. Regards,  [redacted]
