our company insurance requirements. Pure Cleaning Services did not take issue with any of our terms and conditions and proceeded to perform cleaning services at the store on October 8, 2015. Warwick issued check [redacted] to Pure Cleaning Service on November 6, 2015, but could not send the check to Pure Cleaning Service because we had not received the correct, required paperwork. Several requests were made but a correct insurance certificate with the required endorsements was not received by Warwick. After contacting the insurance agent listed on the certificate directly, Warwick was able to obtain the correct insurance certificate (see attached) with the endorsements on November 18, 2015. The check (see attached) was sent to Pure Cleaning Services on November 18, 2015 via [redacted]. This matter has been resolved.
