This care was in response to a referral generated by Dr. Kristine H[redacted] This care was billed toMedicare and subsequently his secondary insurance, Aetna. The total bill for these 6 visits was $810.00or $135.00 per visit. To date Mr. [redacted] has paid $10.00 of his outstanding bill and collection notices havebeen sent. We have given him almost 12 months to respond to this situation. He has received billstotaling $270.00. This amount represents 2 massage visits as this is all Aetna will acknowledge to datepending notification from Medicare. Further bills totaling $540.00 are to follow. Regrettably, thisgentlemen will more probably than not be placed in collection for $800.00 at months end withoutproper arrangements.

These visits were billed at a discount rate of $65.00 per massage totaling$130.00 in which only $110.00 was paid. He has a residual balance of $20.00 dollars for this care. Hehas ignored all bills and notification forms this office has requested.
Enclosed is a copy of our office massage intake form. It states, "I also understand that I am responsibleif third party payment is not made." A signed copy can be produced upon request.
