February 22, 2018Revdex.comHoliday Drive, Suite 220Pittsburgh, PA 15220Regarding: Family Medicine-North MainID# [redacted] Dear Ms [redacted] I am responding to what Dr [redacted] office wroteThere was no indication on the new patients sheet that stated that being late for an appointment is considered a no showI also revised their attached cancellation and no-show appointment policy and there was nothing stating that being late is considered a no-showThe only information on the new patients sheet that refers to “no show” is that they will not rescheduleI was told by the other doctor’s office that the note said “frequent no show”If this information is not true why it is on the Washington health system, this means someone made a mistake somewhere and need to remove itThis note will continue to damage my reputation until it is removedWhy would they even post a note on the Washington health system about a new patient that would affect getting an appointment somewhere else? If I was a patient with this office and I constantly not show up for my appointments then it would be fair for them to post a note on the Washington health system stating my occurrenceIs it possible to get a copy of the note that is on the Washington health system? I do not want every time I try to get an appointment under the Washington health system I have a problem.Sincerely [redacted]

February 7, 2018Revdex.com Holiday Drive, Suite Pittsburgh, PA 15220Regarding: [redacted] Revdex.com Assignment # [redacted] Attention: [redacted] Dear Ms [redacted] :We are responding to the complaint made by [redacted] regarding her failed appointment on 10-16-Ms [redacted] alleges that another physician's office told her that there was a note in the record at our office stating she was "a frequent no show to my appointments"This is not trueThe only documentation in our record is enclosed for your review and is very factual stating:"Patient was about minutes late to her new patient appointment, although she was told earlier in the day when she called to notify us she would be late that we only allow up to minutesShe was advised that we would not be able to reschedule her".Part of the new patient paperwork that Ms [redacted] received prior to her October 16th appointment clearly states that new patients must arrive minutes prior to their appointment timeIt also states that if a patient "no shows" for their first appointment they will not be scheduied(A copy of this is attached.) She had also received an automated voice reminder on October 16th regarding her appointment time and reminding her to arrive minutes prior.Our office tried to accommodate Ms [redacted] and as per our documentation, when she phoned prior to her new patient appointment stating she may be late, she was told she could not be more than minutes late or she would not be seen and would not be rescheduledMs [redacted] arrived nearly minutes late for her appointment (which is considered a no show) and we were not able to accommodate her as it was necessary to see other patients who were on time for their appointments.Sincerely,Dennis O [redacted] MD [redacted]

March 6, 2018Revdex.com 400 Holiday Drive Ste 220 Pittsburgh PA 15220Regarding: [redacted] Case ID [redacted]Attention: [redacted]Dear Ms. [redacted]:I am replying to additional concerns raised by [redacted], reference your letter of 2-27-18.Ms. [redacted] contends that an employee of another practice informed her that we entered a note in her chart stating that she was a "frequent no show". We have looked through Ms. [redacted]'s medical record and cannot find this entry. The only entry we made is included with this letter and is labeled "Exhibit C".Ms. [redacted] also states there is a conflict with our no-show policy. The policy she included in her letter "Exhibit B" is for patients that have been seen and established at our practice. Since she was never seen and established, it does not pertain to her circumstances. Please reference the ''New Patients" letter sent to her in her "Welcome" packet which is "Exhibit A". It clearly outlines the responsibilities of becoming an established patient at our practice. It also clearly states that "our office requires a 48 hour notice if you need to cancel your appointment. If you no show to your 1st appointment, you will not be rescheduled". Because Ms. [redacted] was 30 minutes late for her appointment, she was considered a "no show". We had specific time allotted for her and her only, and we lost an opportunity to schedule other patients in the 30 minute time slot reserved for her. Also, our "New Patients" letter instructs patients to arrive 15 minutes early for their first appointment.Our note dated 10-16-17 is factual and would not prohibit Ms. [redacted] from receiving services from another provider.
