Dr. J[redacted] is a preferred provider for...

United Concordia. This preferred provider status indicates that an agreement was signed between Dr. J[redacted] and United Concordia stating that he will not charge more than what is allowed by the insurance company for the covered services provided. Dr. J[redacted], in his response admits to providing only emergency services but he charged me for all the services he needed to provide including a complete root canal. This totaled 787.00. He then billed my insurance company and my insurance company paid him for the services. Despite contacting him several times and providing my updated contact information his office DID NOT RESPOND to me. They did not contact my emergency contact listed in my paperwork either. Dr. J[redacted] was paid twice for services (some of which I never received) and this is FRAUD.

A Root Canal Service was received by Ms. [redacted]. As I said we contacted the insurance company and they requested repayment. Preformed unnecessary work on my teeth that I did not ask for, including taking x-rays & when asked to release the x-rays I paid for,I was denied.
I went to visit Dr. Wayne Jackman on January 18, 2013 to evaluate the replacement of a tooth that had broke. I did not ask to have my teeth cleaned or examined, I went specifically to see what they could do about the one broke tooth. Once I was in the office, they proceeded to clean my teeth ($55.00), examine all of them ($66.00) and take x-rays of not only my tooth but they also took an x-ray of my entire head ($422.00) which I view to be totally unnecessary. I asked to have my x-rays released in my possession on January 28, 2013, the office assistant told me that I would be able to come and pick them up at noon. Once I arrived at Dr. Jackman's office, I was informed that I would not be receiving my x-rays even though I was told that it was not a problem for me to come pick them up. On January 29, 2013 I had a dental appointment with another dentist. Since my x-rays were not released, I had to pay to get new ones done. The doctor that I went to get my second opinion from informed me that it was completely unnecessary to take an x-ray of my entire head.
