It is always our goal at WellDyneRx to provide our members with the best... service possible. WellDyneRx contacted the member and member's wife in October 2015 in an effort to resolve their issue. We were successful in our attempts. The member's employer group implemented a clinical program to ensure safe and appropriate medication use. The member's medication was limited to a 15 day supply and requests for extended therapy days require clinical documentation from the prescriber. WellDyneRx received an incomplete documentation on September 30th. We sent a fax to the prescribing physician and received additional information on October 1st. The medication request was approved on October 2nd. WellDyneRx's prior authorization team is able to answer members question during normal business hours plus a pharmacist is available 24 hours a day, 7 days a week for emergency situations. WellDyne made several attempts to contact the member in an effort to resolve their issueWe were successful in reaching the member back on July 20th
During this time a staff pharmacist reviewed the requested information and also was available to answer any additional questions
WellDyne's staff pharmacists are available to answer members question during business hours plus they are also available hours a day, days a week for energy situationThey may be reached by calling XXX-XXX-XXXX
In addition WellDyne has also checked its equipment to ensure that all of the information is visible and present on the medication labelA review of the equipment did not reveal any issue with the equipment

Other members would prefer to keep the prescription on file and release it at a later date.
As noted above there was no information requesting that we hold any prescriptions. both phone calls as well as letters all of these attempts have been unsuccessful.  In addition we have contacted the individuals
group and have asked them to reach out to the individual as well.
Unfortunately the information provided in your compliant does not match
our information.  We have also spoken to
the individuals group to ensure that we have the correct information.  The information provided by the group match’s
our information.
