Wellstar OBGYN

I made a preventative insurance covered physical with a Dr. I had seen for a number of years, who changed offices within Wellstar. When making the appointment at the Wellstar OBGYN clinic, still associated with Kennestone Hospital where I had been before, I told the receptionist what insurance I had and if procedures would be covered. She stated yes, any would be covered. I also called insurance to make sure since it was a different office. I went to Dr. December 2018, having only a pelvic exam and getting a referral for mammogram. January 2019, I received a bill for $271.24 for "facility" charge. Upon calling insurance, who totally covered the 10 minute pelvic exam for less than this $271.24 charge, the agent said he had never heard of a "facility" charge, so this was not covered. I contacted this Dr. office many times which I've documented, personally spoken with Wellstar customer relations, as well as the complaint department who agreed that this fee had not been disclosed to me. NO ONE I have spoken to has been able to explain what a"facility" charge is. An agent with Customer Relations personally told me to contact Jennifer Kalb, manager of the facility to clarify this charge. I personally spoke to her who told me it was a new educational facility, and the receptionist didn't know about the charge when I called. She told me it would be refunded to me, since I had paid this to avoid legalities. I never received a refund, nor will she return calls, or see me when I tried to see her twice at her office. I feel I have a right as a patient to know who determined this charge and what exactly a "facility" charge for a doctor's office is. When I asked if all patients were charged this, I was told under HIPPA they could not tell me. I will be glad to provide names of 12 people at Wellstar I have spoken with about this charge. As a patient I feel I have the right to know what exactly this "facility" charge means, how this exact number was determined, and who determined this exact dollar number.
