As the administrator I have only spoken to the patient once after she exhausted efforts speaking to [redacted] ***, billing mgr., [redacted] , collections mgrand [redacted] billing I explained to her that we received no paperwork for any of the visits (please see below her patient records) Dr [redacted] did not leave us – we did not renew his contract We had nothing to do with this clinic – he set this up on his own in a spa (not a doctor’s office) and he DID NOT return paperwork or money from this clinic We balance daily with all payments crossed checked by individuals We balance each day per approved accounting standards I have several questions which MsWallace was unwilling to answer: Why did Ms [redacted] pay $when she had a co-pay of $50.00? Why did she wait months after receiving a statement each month to call us? How did she make the appointment? Did she make a follow appoint and is she continuing to see Dr [redacted] ? She cursed everyone she spoke to even to the point of taking God’s name in vain and I will not tolerate such behavior with my employees I have made every effort to try and help Ms [redacted] but unless she can produce a receipt that is from Western Kentucky Heart and Lung Associates (computer generated – our only type of receipt) there is nothing else I can do for her If you would like to discuss further after you receive this email please feel free to call me Please see below on right hand side there is no copay listed Please note the number of statements sent to the patient before we heard anything from her We did not hear anything from her until she was turned over to outside collection agency

I am starting to feel very irritated, almost like they are taking their anger for Dr*** leaving their practice on me individually. I have absolutely no reason to lie in this matter, if I were going to lie why would I not say I paid the entire copay...not 20$ of it leaving a 30$ balance. I believe they are trying to accuse me of writing myself a hand written receipt and that is absurd!! I have also spoken with Dr*** and he has written and been attached to this responseThere is no logical reason for me to lie regarding anything in this matterI would be happy to have a handwriting specialist analyze my hand writing as well as the receipt that was given to me.
***'s story has changed so much since all this started, first I wasn't a patient (I had no paperwork, no packet filled out, no insurance card on file, NO FINANCIAL agreement signed, etc), then I was a patient and for some reason non of my paperwork made it back to them. Then, I had to find my receipt for the date of service and they would credit my account, well I found the receipt and now that is not good enough. Now I am being accused of handwriting myself a receipt. Seems very strange to me that the story keeps changing and mine has been the exact same

Dr***'s temporary nurse *** (the one that worked in *** *** office with him) did take a 20$ payment and gave me a hand written receipt. She did not have computer access with WKHL and she has stated that they issued to Dr*** a hand written receipt book to use. (Who makes this garbage up??) This story may be confirmed by contacting *** *** office and speaking with *** at 270-***. She has personally contacted *** at WKHL to discuss this matter, again with no successI honestly am beginning to believe that no matter how much proof I give this administrator; she is not going to accept it and dispute the matterI have contacted *** and she has agreed to send me something in writing regarding accepting the payment, as well as confirm writing me a hand written receipt for the date of service as well as using a receipt book instead of what *** has alleged they used.

Dr. [redacted]...

contracted with Western Kentucky Heart and Lung Associates from 8/1/2010 thru 9/6/2013. He conducted his practice at remote locations which he set up and staffed. On 9/6/2013 (his last day contracted with WKHL) he had a clinic in Hopkinsville where he allegedly received a co-pay from [redacted]. (Please note that the receipt is not a WKHL receipt (l have enclosed one) also please note the hand
writing for the receipt and Ms. [redacted]. I explained in detail to Ms. [redacted] that we did not receive her co-pay and thus could
not credit her account for it.
Dr. [redacted] billed this claim himself— see attached and we had no contact with the billing. Ms. [redacted] needs to speak to Dr. [redacted] new office as I have explained to
her.
