To whom it may concern,The date of the surgery was January 8, 2016. The correct and active insurance (insurance was active until 1/31/16) was billed. The amount of $1,that the patient paid was the correct amount due to the patients required insurance deductible. All of this
information is available for the patient to review on the patients insurance companies website under claim status. Per patient the new insurance was active February 1, which was after the date of the surgery
