I am writing in response to the letter we received from the Revdex.com, dated 5/6/2015, in regard to concern number [redacted] .We received this letter here at our Administrative Office last week, and have responded to the concernOur billing office has reached out and spoken to our customer, and she will no longer receive any bills for the visit in June that was of concernThe customer stated that she is satisfied with this resolution.From our internal review, the bills that were sent were correct, based on the initial income documentation that the customer supplied to our office and the federal regulations that we are required to follow for our sliding fee scaleWe were not able to gather the additional documentation that we needed from thecustomer that would have allowed us to adjust these billsHowever, we did apologize to our customer for the multiple attempts that she had to make in order to find a resolution to her concern.If you have any questions, please do not hesitate to contact me at [redacted]
