Complaint ID: [redacted] Dear Ms [redacted] ,This letter is in response to a complaint by [redacted] regarding her ex-husband who was a patient at Wexford Healthcare Center from May until March During Mr***’s stay with us, Mrs [redacted] voiced numerous complaintsShe contacted numerous agencies and lodged complaints which resulted in numerous investigations by these agenciesNone of which could substantiate any of Mrs***’s allegationsDue to the number of complaints Mrs [redacted] voiced regarding the care and services of her ex-husband, the facility assigned a RN Unit Manager as her primary contact person to field and investigate any complaints so that Mrs [redacted] could communicate with one person to eliminate the possibility of miscommunicationWith reference to the resident fund account, [redacted] withdrew $from Mr***’s account on three separate occasions, please see attached documentationOn the patient fund statement, it reflects a withdraw of $for this time periodThis was explained to Mrs***It has also been explained that per the Medicaid regulations, monies from the patient fund account can only be refunded if funeral expense receipts are provided.On the day of Mr***’s death, the RN Unit Manager took the responsibility of packing up Mr***’s belongings in boxes as per the facility’s policyThe family was informed that we were doing this and the son became very upset, claiming that he had personal things amongst his dad’s belongingsThe family immediately came into the facilityMr***’s son was yelling aid swearing at the staff members and-throwing boxes to the point that the police were contacted and he was escorted out of the facilityOn 3/3/14; Mrs [redacted] contacted me regarding a t-shirt that was missingWe attempted to look for the t-shirt but could not locate itShortly thereafter, Mrs [redacted] claimed that there were two missing t-shirts but she could not provide a description of the t-shirtsIn lieu of tryingto find missing t-shirts with little to no description, the facility will reimburse Mrs [redacted] with a money order in the amount of $20.00.During Mr***’s stay at Wexford Healthcare Center, the facility offered more than once to find another facility for her ex-husband as she clearly was not happyMrs [redacted] refused to move ex-husband to any other facility that accepted him as Wexford Healthcare Center was convenient for her and her son to visitIt should also be noted that Mr [redacted] voiced that he did not want to move from the facility.The facility fully believes that we provided quality care to Mr [redacted] during his stay of nearly two yearsIt is unfortunate that we could not please his family.Should you have any questions, please do not hesitate to contact me at ###-###-####Sincerely, [redacted] ***Administrator

Review: My ex-husband that I held power of attorney for was at the Wexford House Nursing Center for almost 2 years. The Business office never gave an explanation of who took money out of his resident money account. On Several occasions $150 per day was taken out of the acct. when a max of $50 per day is permitted. Since I am the only one to have access to withdrawal, they could never explain why or who took the money. I had asked to see evidence of who took the money out and they never could explain, ex: I am new, I have a conference, or they don't know where paperwork is. Also phone calls were not answered or returned. Numerous articles of clothing went missing. They have a replacement policy, but has never been honored. On 2 occasions he was transported to wrong medical facilities. On a consistent basis the rooms floor, bed, and bathroom were not maintained. If the area became soiled, the next shift would clean it or waited to the next day. On the day [redacted] passed away he was at a local hospital. With in 2 hours his possessions in his room (pictures on wall, things in drawers, photo albums, and dvds) were removed and boxed up without the Family's permission. However his cloths in the closet were still hanging. It felt like being violated. My son was upset that someone went through his Dad's possessions and treated it like trash. No compassion or symphony was given. Please note that [redacted] had short term memory lose and the beginning stages of [redacted]'s [redacted].Desired Settlement: I would like a written and verbal expiation of the above complaints. Also lost clothing credited. And a check for the correct balance 'resident money' fund sent to me.
