I do have a response for Mr***'s complaintThe details of the situation are as follows:
Mr*** did
miss an appointment with Dr*** *** on March 30, He did not call to cancel or reschedule the appointmentIt is our office policy to charge patients a $fee when they no-show for an appointmentSo, Mr***'s account incurred a $chargeWe mailed him a statement on April 2, which reflected the feeHe later called and left me a voicemail voicing his frustrations with our policyHe also indicated in the voicemail that he was dealing with his father's death during that time but that he would be mailing us our payment along with a copy of his father's death certificate
I called Mr*** back and left him message to please return my call so that we could discuss the situationMy intent of calling him was to let him know that we would adjust off the $A few days later our office received his payment, a letter, and a copy of his father's death certificateI again called Mr*** and left him a voicemail stating that we would not be depositing his check and that we would be adjusting off the $I also stated in my voicemail that we had not received a signed consent form to release his medical records to another practiceI asked if he could call me back to let me know if he wanted his check returned to him or if we should shred itMr*** never returned my call so I shredded the check
Our office has yet to receive a signed consent form from him to release his medical records to another practiceThis is legally required in order for us to transfer his recordsOur office has the necessary form or the office that he is transferring to will also have this form
