WholeKids Pediatrics is sending this letter to address a complaint made against us to the Revdex.comIn regards to complaint ID [redacted] made by [redacted] on 12/29/we wanted to respond with the following: We do charge a nominal annual fee for our wellness planOur families are made aware of this annual fee at the time of scheduling the first appointmentIn addition, families sign a form that they are agreeing to pay this fee and the services that are providedThe wellness plan has been in effect for the past three yearsIn fact, Ms [redacted] has paid the fee in question in and and has had ample time to discuss this with usWe are within our legal rights to charge this feeOur attorneys were consulted prior to implementing the annual feeIf there are any further questions regarding this fee, we can involve our legal team if needed.As to Ms [redacted] ' complaint about her child's medical records, she was notified that we could not release her child's medical records with a signed consent form to do soMs [redacted] has to date not signed this records release form which was mailed to Ms [redacted] for her conveniencePlease refer to Ohio Revised Code section regarding the fees that are associated with releasing medical records in the state of OhioWe charge a flat fee of $per childThe Ohio Revised code allows for substantial fees related to dissemination of medical records which we felt were onerous for our patients In regards to statements sent to Ms [redacted] , the statements reflect balances that are related to medical services and any applicable fees such as the wellness planWe always have a billing specialist and/or our practice manager available for answering questions that might arise related to balancesAt WholeKids Pediatric, we strive to always put our families and patients firstOur goal is to ensure that our patients receive the best care possible and that our families remain satisfied with their careif concerns arise, we are more than willing to address them and resolve them as judiciously as possibleWe are genuinely sorry that Ms [redacted] has been unhappy with her care at WholeKids Pediatrics Since Ms [redacted] has decided to seek medical care elsewhere, she does not owe the wellness plan for and that will be removed from her accountWe will also expedite release of medical records once we receive her signed consent to do so but the fee to process medical records does still applySincerely, Dhanu S [redacted] **

We do charge a nominal annual fee for our wellness plan. Our...

families are made aware of this annual fee at the time of scheduling the first appointment. In addition, families sign a form that they are agreeing to pay this fee and the services that are provided. The wellness plan has been in effect for the past three years. In fact, Ms. [redacted] has paid the fee in question in 2014 and 2015 and has had ample time to discuss this with us. We are within our legal rights to charge this fee. Our attorneys were consulted prior to implementing the annual fee. If there are any further questions regarding this fee, we can involve our legal team if needed.As to Ms. [redacted]' complaint about her child's medical records, she was notified that we could not release her child's medical records with a signed consent form to do so. Ms. [redacted] has to date not signed this records release form which was mailed to Ms. [redacted] for her convenience. Please refer to Ohio Revised Code section 3701.741 regarding the fees that are associated with releasing medical records in the state of Ohio. We charge a flat fee of $20.00 per child. The Ohio Revised code 3701.741 allows for substantial fees related to dissemination of medical records which we felt were onerous for our patients. 
In regards to statements sent to Ms. [redacted], the statements reflect balances that are related to medical services and any applicable fees such as the wellness plan. We always have a billing specialist and/or our practice manager available for answering questions that might arise related to balances.
At WholeKids Pediatric, we strive to always put our families and patients first. Our goal is to ensure that our patients receive the best care possible and that our families remain satisfied with their care. if concerns arise, we are more than willing to address them and resolve them as judiciously as possible. We are genuinely sorry that Ms. [redacted] has been unhappy with her care at WholeKids Pediatrics. 
Since Ms. [redacted] has decided to seek medical care elsewhere, she does not owe the wellness plan for 2016 and that will be removed from her account. We will also expedite release of medical records once we receive her signed consent to do so but the fee to process medical records does still apply.
