Pt came in for eye exam on 12/12/14 pt had appt on 12/19/14 pt cancelled appt. pt came in on 1/6/15 for eye glasses quick check. Glasses were dispensed too pt on 1/6/15. Pt still has the glasses. pt came back on 2/20/15 eye glass quick check...

then again on 3/10/15 then had appt for 4/7/15 no showed for appt that we could of charged pt for but did not. never heard anything from pt until 8/17/15 for which she cancelled appt because we sent her to collections from which we sent pt statements on 1/14/15 2/18/15 3/31/15 5/19/15 and then on 7/8/15 we sent pt to collections We have tried to accommodate this pt back have not had any success.

Lenses with incorrect prescription was given to me. I informed Wight Eye Care that I was not paying until I received the correct prescription. Dr. Wight was unsure why he could not come up with the right prescription. I canceled my eye exam on April 7, 2015. I notified them that I could not make that appointment. What accommodations? I still do not have the right prescription. Three weeks prior to my August 17, 2015 appointment, Wight Eye Care said they would take care of the collection. The day of my appointment, there was no noticed that they took care of my collection, so therefore I canceled my appointment due to them not following up on their word. I was also told by the receptionist when the office manager got there, she would call me. I never received a phone call from the office manager.

Yes pt did receive a phone call from myself. I spoke with the pt told her she had to come back in and get a recheck with the Dr. and then I will take out of collections like I said in the first email we have several times tried to accommodate the pt but she cancels appointment or no shows. The pt has the glasses but has NOT PAID for them so therefore I will NOT take out of collections pt rx is changing and Dr. can't make a decision is pt rx is changing every week so there no other lenses were made.

I have visited Dr. Wight three times within the past year and each time Dr. Wight comes up with a different prescription for corrective lenses for me. The last time I was in which was on March 10th, 2015, Dr. Wight told me I must have something wrong with my immune system, and that I should wait to do another exam. As a Registered Nurse, this frightened me. I had a complete eye exam with Dr. Malik on April 15th, 2015, and he said there was nothing wrong with my eyes. I saw my primary care physician on June 15th, 2015, and had a complete check-up including blood work, and my physician said nothing was wrong. I also saw my OB/GYN and had a complete check-up, and again nothing was wrong. I have documentation for all my exams. Dr. Wight is the first eye doctor in 42 years that cannot come up with the correct lens for me, but I'm supposed to pay for incorrect lenses? This is medical fraud. I have never been a no call or no show. As a professional, I would not do this. I called to cancel my appointments back in April and in August of 2015. With my insurance through Northern Illinois Health Plan, I only have one exam allowed for every two years. I feel Dr. Wight should reimburse me and the insurance company for the eye exam because he has not been able to complete a correct exam. I'm filing a complaint with the State of Illinois Department of Financial Professional Regulation. The incorrect lenses were returned by certified mail on September 25th, 2015.
