Paid in advance $5250 31 days for wife ***) to stay at Willow Springs. 31 days covered 8 November through 8 December. During wife's short stay at nursing facility, wife had to be transported twice to a Hospital Emergency room. This resulted in wife's transfer to another nursing facility on 23 November.
Requesting reimbursement for unused days (24 Nov - 8 Dec) which comes to approximately $2550. Also, requesting billing statement/invoice for the time wife was at Willow Springs.
