To Whom It May Concern: Patient was new to me and requested initial visit be scheduled as a wellness exam which is paid 100% by his $10,high deductible insurance planIf his issue was only wellness and there were no other patient complaints or physician concerns then that would have worked out
to his advantageHowever, he had multiple complaints or issues of which only were listed on his intake paperwork and the other were separate patient complaints or identified issues by myselfHe did not stress to me that he was unprepared to delve outside of the wellness designation of his appointmentHe may have done that with my office staff but they are not in position to make clinical decisions regarding final visit statusHis multiple medical issues were clearly documented in my note (total of diagnoses) and all essentially required some documented intervention or recommendation The patient complaint filed with Revdex.com notes he says I stated it was only the hypertension that prevented me from billing this as a wellness exam but that is only partially trueAfter gathering the history, doing the examination, and discussing my concerns and recommended treatment or observation options it was apparent I would not have time to do the multiple components of a wellness exam in addition to the new patient medical evaluationIf I could have done the wellness components along with the additional medical evaluation he still would have had charges that could not be covered under his wellness examinationIf I resubmit the current documentation with a wellness diagnosis but no documented wellness components his insurance will deny the claim and he would be responsible for the charges stillI can't manipulate the note in retrospect to fit the desired charge as it would be considered fraudulentIt is unfortunate I was not aware of his earnest financial concerns as he is semi-retired and on Social SecurityHowever, I did not force him to offer his medical complaints that needed to be evaluated and addressed as a new patientI did not choose his insurance plan either and cannot be held responsible for those financial consequences though I generally try to work with patients when possible if they make it known to me during their visitEnclosed is copy of the conversation I had with patient after he called my office on 4/28/I'll admit, I didn't realize his concern for the $10,high deductible insurance until after our conversation when I tried to understand why he was upsetIn summary, my performance as a physician with this new patient was completely appropriate although he had other expectationsAgain, I cannot alter his current documentation as that is unethical and illegalIf he would like to discuss an adjustment to his charges my billing office can work with himThank you for your time
