records and have arrived at the following:· ** did have an appointment scheduled for 10/06/2017, as he indicated· ** is aware of the late cancel / DNS policy. He signs documentation annually, and has had multiple missed appointments in the past (fees waived as courtesy on a few and then fees paid)· On 10/5/17 NQ called both **’s on file home and on file cell numbers to confirm the appointment, documenting in both cases that she could not leave a message as mailboxes were full. Calls were made at approximately 11:26 AM.· On 10/6/17, 20 minutes after the appointment time, RR marked the appointment as missed and applied the DNS fee (note that this was the third try at an appointment, ** had already rescheduled twice).· On 9/8/17 pt had received statement with fee for a late cancellation or 8/31 appointment. On 10/6/17 an invoice was sent for the two fees. · On 11/3 patient made payment for one of the fees and included a note requesting to waive the newer one as he believed he had left a message. The letter was scanned in by RR, but it appears no action was taken.· On 11/6 and 12/6 subsequent statements were sent out. On 12/18 the encounter charges were deleted by RR. RR was the front office supervisor and I believe the person with whom ** was working.** has a $0 balance on his account. Nothing has been sent to collections.
