Wise Family Practice Urgent Care (WFP) is in receipt of your notification of the complaint made by [redacted] , which is assigned ID # [redacted] The facts of the financial responsibility of [redacted] is that $was owed to WFP as a previously balanceUnder the contractual agreement [redacted] has with her commercial insurance company, she owed a $co-pay for serviced rendered during her appointment on 09/30/On October 1, 2016, [redacted] spoke with two WFP representatives and it was offered that the $co-pay that she owed for her 09/30/office visit would be refunded; however, it would be billed at a later date [redacted] seemed confused and did not understand that she still owed $as outlined in her contract with her insurance companySaid representative of WFP apologized multiple times for any confusion during her being checkfor her appointment as to the previous balance she owed ($20.40), as well as the co-payment she owed $20.00, totaling $In conclusion, services were provided to [redacted] and she will be billed for the the co-pay due of $20.00, which was payable at the time of serviceThank you for the opportunity to respond to this complaintIf you have any questions, please feel free to contact my Accounts Payable Manager, [redacted] at [redacted] ***

has with her commercial insurance company, she owed a $20.00 co-pay for serviced rendered during her appointment on 09/30/2016. On October 1, 2016, [redacted] spoke with two WFP representatives and it was offered that the $20.00 co-pay that she owed for her 09/30/2016 office visit would be refunded; however, it would be billed at a later date. [redacted] seemed confused and did not understand that she still owed $20.00 as outlined in her contract with her insurance company.
