Statement by Mr [redacted] is correct in that we did not receive a request to cancel the DeltaCare Plan until he called us on...
November 5th 2015. Per our conversation, he has a note to fax us and our fax number on the note and a recollection that it was in April 2014 but we never received a fax to cancel the plan. He states that the premiums were being drafted from his fathers account and that he did not see those statements showing the monthly continuing premium draft.
We cancelled back to XX-XX-XXXX even though the terms of his coverage require he give us a 30 day notice and premium for November was collected. We are in the process of refunding the November premium to him.
The monthly plan premium was forwarded to the carrier, DeltaCare, with a portion going to his elected dental provider for his coverage under the HMO dental plan.
It is unfortunate that this situation occurred but we cannot refund the retroactive premium.

My issue goes unsolved. Its unfortunate that my dues are non refundable even though I cancelled. At this point I just have to take it as a loss. I appreciate the response. Thank you
