Dear Sir/Madam,We have not participated with *** Insurance for the past five yearsOur staff are well aware to the fact and would not tell anyone that we participateWe however accept the PPO option of the insurance since they do have out of network benefitsThe patient was coming in for a problem visit and was aware of the copay of $our policy as I have attached a copy indicates that the copy has to be paid at the time of the visit or there will be a charge.We cannot send any records without a written authorization from the patientI have told the patient this fact and since we never received any written request the record was not releasedThe patient has told me multiple times that the check for $is in the mail but to date we have not received the checkThis account is due to go to collection and I advise that she pays promptly.Thank you and you should have any questions please do not hesitate to contact me at ###-###-####Sincerely,Reza J***
