Initial Business Response / [redacted] (1000, 8, 2015/04/22) */ April 17, Revdex.com of MN and ND South River Ridge Cir Burnsville, MN Dear Mis, [redacted] , I am writing in response to a correspondence I received from the Revdex.com of MN and NDThe correspondence is in regards to a complaint submitted to your office by the daughter of a former resident of Woodlyn Heights Senior Living During our facility review of this complaint, we were able to substantiate that the resident had in fact elected Health East Hospice as the provider for her care while she was still a patient at the hospital Based on the information available to us it appears that the private pay financial obligations associated with making this hospice election was not clearly explained to the resident or family member by either the hospice provider or hospital staff prior to her discharge As a facility, it is our mission to provide services and support to our residents and families from the point of admissionOur social service staff make it a priority to meet with residents and family members as soon as possible after admitting to Woodlyn Heights to ensure questions and concerns are addressed timelyIt is also at that time the admission paperwork is discussed, completed and signed by the responsible party Please do not hesitate to reach out to me if you have any additional questions Sincerely, [redacted] , LNHA Executive Director Woodlyn Heights Senior Living

I am writing in response to a correspondence I received from the Revdex.com of MN and ND. The correspondence is...
in regards to a complaint submitted to your office by the daughter of a former resident of Woodlyn Heights Senior Living.
During our facility review of this complaint, we were able to substantiate that the resident had in fact elected Health East Hospice as the provider for her care while she was still a patient at the hospital.
Based on the information available to us it appears that the private pay financial obligations associated with making this hospice election was not clearly explained to the resident or family member by either the hospice provider or hospital staff prior to her discharge.
As a facility, it is our mission to provide services and support to our residents and families from the point of admission. Our social service staff make it a priority to meet with residents and family members as soon as possible after admitting to Woodlyn Heights to ensure questions and concerns are addressed timely. It is also at that time the admission paperwork is discussed, completed and signed by the responsible party.
