Mr. [redacted] last payment on his account was 5/9/2013.   A monthly statement was mailed to him on June 1st.  A 30 day delinquency notice with his monthly statement was mailed on July 1st, 2013.  A 60 day delinquency notice with his monthly statement was mailed on...

August 1st, 2013.  On August 18th, 2013, Mr. [redacted] mail was returned from [redacted] 
[redacted]  We called the phone number on file for Mr. [redacted] and spoke to his grandmother who stated Mr. [redacted] had moved to ** and she did not have a contact phone number or address. We send a final notice when the patient is over 90 days delinquent, however we did not have a forwarding address or contact phone number for Mr. [redacted], therefore his account was turned over to Collections on September 3rd, 2013.Please feel free to contact me if you need anything else!Thanks,[redacted]Wright Dental Associates
