to hear that Ms. [redacted] complained about the fees charged on medical services provided to her. We appreciate your office communicating this situation with us.
Ms. [redacted] came in on 01/31/2016, she did not have any medical health coverage. She paid out of pocket for the medical services provided in the amount of $150.00. She came back on 03/20/2016 and had [redacted] for her Medical Health insurance. Her medical benefits stated that she has a $4,500 individual annual deductible and [redacted] will cover 60% after satisfying her individual annual deductible. Our office billed E&M 99.214 $171.40 and cpt 99051 $53,44 for after hours and/or weekend per allowed Medicare Fee Schedule. [redacted] allowed $127.72 and was applied towards her deductible. Ms. [redacted] has a credit in our office in t paid for in our office. -
Ms. [redacted] has addressed her concerns with her Medical Health Insurance Carrier ([redacted]) last May 2016 and the same explanation was provided to [redacted].
Please accept our sincerest apology for any trouble or inconvenience this may have caused. We hope that this may have clarified and explained the fees Ms. [redacted] is addressing.
