Ms*** was notified that the death certificates would have been picked up on Friday 5/26/She was notified she could come by and pick them up or they would be mailedMs*** left a message on 5/26/indicating to leave the death certificates to be picked upI assumed by the
message that she was requesting the certificates be left at the mortuaryWe do not have a practice of leaving death certificates at Vital recordsI tried to explain to her the misunderstanding she requested that the certificates be mailedThey were placed in the mail to her home address on 5/26/We also included a letter and a check for $explaining that we charge a little bit more when death certificates are picked up from vital recordsI received a phone call from Vital Records regarding Ms***'s conduct in their officeShe should have the letter, check and certificates at her home by now
