[redacted] , Our intent is never to discharge a patient or to have a patient choose to go elsewhere We work hard at providing quality medical service while remaining compliant with Evidence-based Best Practice recommendations and insurance guidelines We were disappointed when you stated you were going to see a different provider if your PCP here would not write the letter of medical necessity you requested She was unable to write that letter based upon your individual documented history, but did offer to write a letter of recommendation based upon your strong family history of __________ After telling the medical assistant you would find a different provider, you were sent a “You Fired Us Letter” You were not sent a discharge letter The whole household is affected when either of these letters are sent Both of these letters still provide days of service for medical necessity When we spoke, you were informed your provider agreed to complete the next round of vaccines coming due for your child I really thought we made progress during our conversation and were going to be able to come to a resolution We are sorry you didn’t feel the same way

Dear Ms***,
This letter is to confirm your decision to seek medical treatment elsewhere We are sorry that we could not service your needsOur office strives to give the best medical care within our reach
As this was your decision , I would like to inform you I will no longer be the
physician for you and your family and will stop providing medical care to you effective days from the date of this letter
I will continue to provide emergency medical care to you and your family for days while you seek another physician
I suggest you consult your insurance carrier , the internet or the yellow pages of your telephone book as soon as possible so that you may find another phys:can who will assume responsibility for your care
I will be pleased to assist the physican of your choice by sending him or her a copy of your medical records

Complaint: ***I am rejecting this response because:
If it is never your intent to "discharge a patient or have a patient go elsewhere", then why did you do exactly that per our conversation January when I asked to remain a patient with you on a recorded line? I will still happily go to your clinic as I stated in the above conversation you recorded. I got a second opinion, as I stated to your nurses and I have the evidence based practice to back up why I requested the test (UpToDate "Screening for Breast Cancer: Strategies and Recommendations. Dec, 2016, and UpToDate "Management of patients at high risk for breast and ovarian cancer" Dec, 2016). Based on your response, your claims do not match what was stated in our last conversation, in which my provider did not want to reinstate me as a patient based on my Revdex.com complaint (which is highly unprofessional and unbecoming a Medical Doctor). If this is not the case, then provide documentation me and my family are reinstated into your clinic and this topic will be resolved.

Our liability company has advised us not to respond in a public format confirming or denying whether someone is a patient here or not and if they are a patient here again not to respond publicly to a complaint. All patient complaints should be directed to

you were going to see a different provider if your PCP here would not write the letter of medical necessity you requested.  She was unable to write that letter based upon your individual documented history, but did offer to write a letter of recommendation based upon your strong family history of __________. 
After telling the medical assistant you would find a different provider, you were sent a “You Fired Us Letter”.  You were not sent a discharge letter.  The whole household is affected when either of these letters are sent.  Both of these letters still provide 30 days of service for medical necessity.
When we spoke, you were informed your provider agreed to complete the next round of vaccines coming due for your child.  I really thought we made progress during our conversation and were going to be able to come to a resolution.  We are sorry you didn’t feel the same way.
